Job summary

St Elizabeth Hospice is an independent charity and hospice, which improves life for people in the East Suffolk, Great Yarmouth and Waveney areas living with a progressive or life-limiting illness. Established in 1989, the hospice has built a reputation for delivering high-level care to patients and the community it serves. Through medicine and therapy, they ease pain; give life purpose and make life liveable. Last year the charity supported over 4,000 patients and their families.

Our Community Care Coordinators work alongside our clinical teams to coordinate patient services in our palliative care coordination hub. The team covers all aspects of work in the hub including being the first point of contact for callers to our 24/7 advice line OneCall.

The post holder will act as a Team Lead for the Community Care Coordinators in a managerial position but also acting as a Community Care Coordinator covering shifts and providing support to our Virtual Ward service.

The hospices work is centred on the individual's needs and they provide specialist support wherever it is needed; whether that is in the home, another setting in the community, in hospital or in the hospice itself. Support is provided to patients and their families via the In-Patient Unit, the Community Care Unit including outpatients clinics, home visits from the community nursing, medical or therapy team, the OneCall 24/7 telephone helpline and bereavement support.

Hours: Full time 37.5 hours per week - 5 x 7.5 hour shifts which can include weekends and bank holidays

St Elizabeth Hospice provides planned, reactive and coordinated care to patients requiring palliative and end of life care. The community care coordinators, cover 7 days per week, providing support to our integrated community service including the OneCall service.

The post holder will act as a Team Lead for the Community Care Coordinators in a managerial position but also acting as a Care Coordinator covering shifts and providing support to our Virtual Ward service.

KEY ACCOUNTABILITIES

Team Lead

To lead and manage the Community Care Coordinators to include 1-1s, appraisals, performance concerns, absence requests and deal with any line management issues.

To recruit, onboard and provide training for new starters to the team.

To develop and manage a robust rota for the Community Care Coordinators and Community Healthcare Assistants covering days and nights.

To ensure that adequate cover, to the established service model, is provided 8 am to 8 pm, 7 days a week to the OneCall service.

To provide ongoing training and refresher training sessions as required.

To have an overview of the Virtual Ward service, including rostering of care visiting and updating associated systems.

Review and confirm any timesheets and mileage claims for the Virtual Ward team.

To have an understanding of all care coordinator functions to be able to support when needed.

Ensure that StaffCare is up to date with sickness/shifts swaps for both the Community Care Coordinator team and Virtual Ward Community Healthcare Assistants, running sickness reports as required.

To signpost and provide information as and when required. To be a resource and knowledgeable of services and informed of capacity within care providers.

To coordinate visits by our Community Healthcare Assistants, providing care for new patients and to manage existing patients.

To assist in the investigation of administration incidents as directed by the community service lead

Act a role model to the team, positively implementing changes to processes and ensuring high quality service delivery

To attend relevant meetings as required.

To recognise indications of staff stress and to facilitate staff support.

Monitor stock levels for both Community Care Coordinator and Virtual Ward Teams, liaising with suppliers and placing orders.

To participate in the induction of new staff

Community Care Coordinator

To understand and organise care packages for end of life patients e.g. number of carer visits in a day using the software provided.

To support care coordinator role and duties for OneCall, virtual ward and general administration as needed

To maintain patient records, ensuring that all activities e.g. telephone calls, are entered in a timely manner and are accurate.

To ensure patients, families and healthcare professionals are informed of changes and updates.

Ensure adherence with relevant Infection Prevention and Control policies and procedures.

To be involved in appropriate education opportunities relevant to the Hospice.

General

To be able to deal with sensitive information at all times.

To devise with the line manager, a personal development plan in line with the Hospice objectives and personal needs.

To maintain and optimise personal professional skills.

To participate in professional placements and lay person visits to the Hospice, as requested.

To recognise the value of volunteer contribution and play an active part in their support and development where appropriate.

To give feedback from attended education programmes/study days to the multi-disciplinary team.
