Established
in 1989, the hospice has built a reputation for delivering high-level care to
patients and the community it serves. Through medicine and therapy, they ease
pain; give life purpose and make life liveable. Last year the charity supported
over 4,000 patients and their families.
The
hospices work is centred on the individual's needs and they provide specialist
support wherever it is needed; whether that is in the home, another setting in
the community, in hospital or in the hospice itself. Support is provided to
patients and their families via the In-Patient Unit, the Community Care Unit
including outpatients clinics, home visits from the Community Nursing, Medical
and Therapy Teams, the OneCall 24/7 telephone helpline and bereavement support.
Main duties of the job
We are seeking an experienced Registered Nurse to
join the Planned Service within our Community Nursing Team on a permanent
basis. You will work with the Clinical
Nurse Specialist to manage a caseload of patients within the allocated
Integrated Neighbourhood Team (INT). You
will provide specialist
palliative nursing care to patients and their families, ensuring their needs
are holistically assessed, their care is planned to meet their needs and
evaluated.
To

and of working within a multi-disciplinary team. This is a chance to make a real

Hours:7.5
hour shifts Monday to Friday 08:30 to 16:30.

JOB SUMMARYTo work with the Clinical Nurse Specialist to manage a caseload of patients within the
allocated Integrated Neighbourhood Team (INT).
Provide specialist palliative nursing care to patients and their families, ensuring their needs
are holistically assessed, their care is planned to meet their needs and evaluated.
SAFEGUARDING RESPONSIBILITIES

KEY ACCOUNTABILITIES
- To undertake in-depth holistic assessments of patients with palliative care needs,
regardless of their diagnosis and plan care to meet patient outcomes. This includes the
triaging of referrals ensuring all triages are completed in line with the agreed standard
operating procedure.
- To provide information, advice, psychological and spiritual care to patients, their families
and carers, liaising and referring on as necessary to appropriate healthcare professionals
in the Hospice, Hospital and community.
- Manage a caseload of patients, under the caseload supervision of the Clinical Nurse
Specialist, offering continuity of care to patients and families. Whilst allocated to an INT,
work flexibly to ensure planned care work is covered across the entire area.
- Carry out visits to patients and families at their home or within the community care unit.
- Attend and participate in the Interdisciplinary team meetings, presenting patients as
needed.
- Provide support to patients on the Virtual Ward ensuring their specialist palliative care
needs are met.
- Use advanced communication skills in delivering highly emotive information clearly,
sensitively and unambiguously with staff, patients, families and professionals by face to
face, telephone and/or electronic means.
- Provide skilled and expert palliative nursing interventions, working closely with medical
and other colleagues to ensure a coordinated and effective service across boundaries of
inpatient, community and outpatients and across professionals, carrying out reactive visits
where this supports coordination or continuity of care.
- Maintain and develop working relationships with community and hospital colleagues
ensuring effective communication so enhancing patient care.
- Involve patients, families, and carers in co-production of and making informed decisions
regarding strategies to manage their own health and wellbeing .
- Identify own personal development needs, proactively seek feedback, and take effective
action to address them using a mix of strategies.
- Share knowledge, experience and expertise with nurses and others at all levels, through
strategies such as formal and informal teaching, facilitated reflective practice, skills
