Job summary

In March 2023, St Elizabeth Hospice merged with East Coast Hospice to form St Elizabeth East Coast Hospice, with all service provision in Great Yarmouth and Waveney coming under the St Elizabeth East Coast Hospice name.

This is an exciting time for St Elizabeth East Coast Hospice, as we are developing an achievable and sustainable palliative and end of life care service for the residents of Great Yarmouth and Waveney. Its a time of expansion, development and investment in the Hospice Team. We are offering an opportunity to be part of the Community Specialist Palliative Care Team and also work within our Community Care Unit influencing the holistic care of patients and carers with end of life and specialist palliative care needs. This is a chance to make a real difference to peoples lives.

Main duties of the job

We are looking for a compassionate and experienced Registered Nurse to join our dedicated Community Team to support informal carers. This exciting role will work within our multi-disciplinary team to provide guidance, emotional support and practical advice to unpaid carers and families looking after loved ones with specialist palliative care needs. You will play a key role to ensuring informal carers feel supported and empowered to navigate care responsibilities whilst maintaining their own wellbeing. You will have a passion for supporting carers and improving outcomes for people providing unpaid care in the community.

St Elizabeth Hospice continues to drive forward innovation and development of services, both in the Ipswich and East Suffolk locality and further up the coast to the Great Yarmouth and Waveney area. In 2019 NHS Norfolk and Waveney CCG awarded the specialist palliative and end of life care contract to East Coast Community Healthcare. The contract includes a specialist palliative care service which will be delivered in partnership with St Elizabeth Hospice. Our most recent Care Quality Commission report rated the hospice as Outstanding.

Responsible for and manage a caseload of patients offering continuity of care.

Apply clinical reasoning to individual presentations and complex situations, utilising information from different sources to make timely, appropriate, and evidence-based judgements.

Act as a key role within the Primary Care Hubs (PCH) taking a population health approach to assist in the management of carers in the end-of-life population within the PCHs.

Use advanced communication skills in delivering highly emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face to face, telephone and/or electronic means. This may be where there are barriers to understanding.

Provide skilled and expert palliative nursing interventions, working closely with medical and other colleagues to ensure a coordinated and effective service across boundaries of inpatient, community and outpatients and across professionals.

Maintain and develop working relationships with community and hospital colleagues ensuring effective communication so enhancing patient care

Involve patients, families, and carers in co-production of and making informed decisions regarding strategies to manage their own health and wellbeing.

Participate in the development and updating of practice policies/guidelines and procedures using high-quality contemporary evidence-based resources.

Participate in the evaluation of significant events, capturing the learning from these experiences to improve patient care and service delivery.

Identify own personal development needs, proactively seek feedback, and take effective action to address them using a mix of strategies.
