Job summary
We have an exciting opportunity for an Advanced Clinical (Nurse or Paramedic) Practitioner to join the
North West Leicestershire GP Federation to support the Primary Care Network, as part of our Nursing or Paramedic Team. The post holder will be supported and mentored by our Senior Advanced Nurse Practitioner or Senior Paramedic and Leadership Team, and work as part of a multi-disciplinary team
in a patient-facing environment. The post holder will be employed by
North West Leicestershire Federation and be based in a practice within our PCN.
Main duties of the job
The post holder is an Advanced Clinical Practitioner (ACP)
who, acting within their professional boundaries, will provide care for the
presenting patient from initial history taking, clinical assessment, diagnosis,
treatment and evaluation of their care. They will demonstrate safe, clinical
decision-making and expert care for patients. Experience of online
consultation, minor illness and women's health (HRT treatment, contraception, gynaecological
care, etc.) is essential for this role.
They will work collaboratively
with the multi-disciplinary team across all member PCN practices, to meet the
needs of patients, supporting the delivery of policy and procedures, and
providing leadership as required, seeking
quality improvement, and manage some aspects of the quality and outcomes
framework to enhance services. In order to work at this level, NMC or
HCPC requirements for advanced practice must be met.

lifestyles, disease prevention and health promotion.
The
postholder will maintain, update, and monitor accurate systematic patient
records of patient interactions, clinical care provided including adverse
events. You will also lead and ensure concise and
accurate PCN and Federation audits are conducted.
The post holder will be required
to undertake any other duties as deemed reasonable within the PCN and
Federation to support the adherence of the PCN DES Contract.

- Mandatory registration with NMC or HCPC (with no current issues under investigation) which includes revalidation.
- Degree in Adult Nursing or Level 6 qualification / Bachelors Degree in Paramedic Science (or equivalent).
- Currently completing or have completed a MSc Advanced Clinical Practice).
- Independent prescriber or working towards prescribing qualification.

- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
