care, St Andrews Hospice is a highly successful and forward-thinking charity
with a turnover of c£5m, offering both adult and childrens services. With an
excellent reputation both in the local community and beyond, we reach over 800
patients and their families in Northeast Lincolnshire and our wider catchment
area of Lincolnshire, Hull and East Yorkshire for our childrens services.
This role
sits within a multidisciplinary team, working alongside social workers,
childrens family support and bereavement practitioner, spiritual care lead and
other clinical colleagues.
Main duties of the job
An exciting opportunity has arisen for a compassionate and committed professional to join the Family Support Team at St. Andrews Hospice in providing excellent support to adults and children with life-limiting conditions, as well as to their families and carers.
The Family Support Caseworker will play a key role in delivering holistic support to adults and children both within the hospice and across the wider community. The post is central to addressing psychological, emotional, social, and practical needs through all stages of illness, end-of-life care, and bereavement.

Qualifications
Essential
- Minimum Level 3 qualification in Health & Social Care, Counselling, or a related field.
- Relevant training in providing psychosocial support, counselling, bereavement support or related.
- Record of ongoing professional development.
Desirable
- Degree in Social Work or Counselling.
Experience
Essential
- Demonstrable post qualifying experience.
- Experience in carrying out assessments, care planning and review.
- Experience supporting individuals and families in distress.
- Experience working in multi-disciplinary settings.
Desirable
- Lived experience of dealing with a life limiting condition and the ability to use this insight in a professional capacity.
- Experience of working in a hospice, palliative or end of life care setting.
- Experience in social prescribing.
