Job summary
The role involves working directly
with the practices and the multidisciplinary team within the PCN. The role is
pivotal in ensuring all patients receive the best possible care and services
from the GP Practices members.
The Care Coordinators role will
support the member practices in coordinating all key activities including
annual reviews, access to services, advice, and information, and ensuring
health and care planning is timely, efficient, and patient-centered.
The role will include supporting
digital initiatives such as increasing the uptake and use of the NHS App and includes responsibilities for the co-ordination of the
patients journey through primary care.
Main duties of the job
1.
Works within the primary care
team, contributing to leadership of service evaluation and research to promote
quality improvement activity.
2.
Works with the Practices
Patient Participation Group to support communication with the wider patient
community.
3.
Collaborates with other members
of the team, patients and their carers when managing and coordinating care.
4.
Uses healthcare technologies to
optimise service delivery, patients access, and continuity of care

support local general practice, to enable the delivery of high-quality care to
local people. We are in the very fortunate position where 4 of the Primary
Care Networks (PCNs) in Redbridge are working together as part of the
Redbridge-wide GP federation Healthbridge Direct.
This means we can build strong clinical teams within and
across the PCNs. We are now at the very start of an exciting journey of
building a team of over 100 additional clinical roles to support and enable the
delivery of services within each of our 4 PCNs. We are building teams within
and across the PCNs to enable peer support and team working both within and
across professions.

The Care Coordinator
responsibilities include but are not limited to the following:
1.
The focus of the role will be to provide admin support
directly to GP practices within Marshalls PCN.
2.
To work with the GPs and other primary care
professionals within the practice to provide support for the call and recall of
patients on long term condition registers for the Quality and Outcome Frameworks
and Local Enhanced Services and PCN Directed Enhanced Services ensuring targets
are met.
3.
To support the practice in promoting Public Health
Initiatives and Campaigns.
4.
To actively support the practices activities around
immunisations including the delivery of childhood immunisations, influenza and
covid vaccination programs.
5.
To support practices in the identification of
carers. To signpost and provide support and information.
6.
To support practices in contacting the
hard-to-reach patients and those that do not respond to communications.
7.
To support patients to utilise decision aids, help
create single personalised care and support plans, in line with best practice.
8.
To help people to manage their needs through
answering queries, making, and managing appointments
9.
To provide coordination and navigation with the aid
of digital tools for people and their carers across health and care services.
