Job summary

There is an exciting opportunity to join Fairlop Primary Care Network. The role of the PCN manager is to enable the PCN to deliver change, build relationships with local stakeholders, and meet all the requirements of the PCN DES. The post holder will be working directly for the Clinical Director of the PCN and be based primarily within the PCN, but will also be part of a team of PCN managers supporting the Primary Care Networks

Main duties of the job

The Primary Care Network Manager role will directly support the individual Primary Care Network. The post holder will work directly with the PCN Clinical Director and member practices to support and facilitate the co-production and mobilisation of services in line with Network Contract Directed Enhanced Service specifications, and any other initiatives the PCN is pursuing. The post holder will facilitate the development of cross sector partnership working between the PCN and other key stakeholders including commissioners, community services, acute trusts, local councils, mental health, community groups and the 3rd sector as well as other organisations as relevant e.g. housing, education, voluntary, etc. to support delivery of the Network Contract Directed Enhanced Service (DES). The postholder will be working directly for the Clinical Director of the PCN and be based primarily within the PCN, but will also be part of a team of PCN managers supporting the Primary Care Networks in Redbridge.

We are looking for candidates who have demonstrable experience of successful project delivery, experience of working with clinicians and ideally in primary care, are good at building relationships and at working with a wide range of stakeholders, and are able to make change happen.

Working with practices to implement change: Support the recruitment and successful integration of the new PCN roles into the network, ensuring there is clarity of how they will work and operate across practices Work with the PCN practices to identify areas of common concern or potential for collaborative working to improve the resilience of general practices, sustain local 2 services and support recruitment and retention of key members of the primary care workforce Work with the PCN practices to develop innovative schemes, using best practice, to meet PCN objectives, exercise initiative and problem solving within the remit of the project. Directly support the roll out and delivery of new services. Co-ordinate resources in order to formulate, mobilise and ensure the ongoing implementation of plans Manage the planning and reporting of services across the PCN in a cohesive way, ensuring all benefits and efficiencies are identified and realised. This will involve clearly outlining milestones, risks and outcome measures and ensuring that effective plans, processes, quality assurance and governance structures are in place Undertake audits and surveys as required to enable and support project and PCN delivery Identify and develop appropriate tools and measures of improvement to support effective change, e.g. performance frameworks, population health data, outcome measures Support the implementation of relevant and appropriate data sharing agreements, as required by the PCN System working and relationship management: Manage and maintain credibility with member practices and all key contacts within the network, to foster a culture of collaboration and joint working between practices Develop and deliver a PCN communications strategy to ensure the PCN work has high visibility with member practices and local stakeholders Develop strong relationships and engage with a wide range of stakeholders with both clinical and non-clinical backgrounds across and beyond the PCN geographical footprints, to ensure collaborative approaches to service delivery. Build supportive relationships with the other PCN managers and the HBD team Represent the network at meetings, in the community and to the media as required Financial Management: Oversee the management of HBD PCN finances, keeping track of committed funds and budgets, with support from the HBD finance team Bid for new money and funding streams as they become available to the PCN Ensure financial risks are recognised and appropriate action taken Strategy and Leadership: Operate alongside the PCN CD as a key member of the PCN management team Work with the PCN CD to clarify objectives and priorities for network delivery in line with operational realities, financial parameters and contract requirements Keep updated with national, regional and local NHS Policy as it relates to PCNs ensuring the PCN maintain compliance and exploit opportunities for new funding streams and services Identify best practice nationally, and use experiences across the country to inform local delivery CONFIDENTIALITY The post holder may have access to confidential information relating to patients, their carers, practice staff and other healthcare workers.
