Job summary

The Healthcare Assistant for Physical Health in SMIs role includes supporting and complying with processes, procedures and assisting in achieving the Physical Health checks in SMI targets, which should be undertaken each year on all patients on the serious mental health register within General Practice.

The physical health checks will be delivered within GP Practices and other venues, including the patients own home across the Dartford Gravesham and Swanley area therefore access to a suitable vehicle and the ability to travel to a range of venues and settings is essential. You will also be expected to support this cohort of patients by giving health advice face to face as well as signposting and cross-referring to other support services.

We are looking for someone who has experience of delivering NHS Health Checks (albeit training can be given) is competent in undertaking phlebotomy and is able to evidence an understanding of Public Health initiatives and programmes.

Main duties of the job

The key aims of service delivery are as follows:

Assisting in the provision of care that advances health equality in order to reduce the premature mortality experienced by people living with SMI.

Supporting in the development and promotion of a local comprehensive model of care to support the physical health needs of people living with SMI across primary, secondary (acute and mental health) and voluntary care to optimise their physical health.

Supporting the provision of a dedicated service to deliver effective SMI annual physical health checks in conjunction with other providers resulting in an improved experience for those receiving the checks.

Supporting the development and provision of outreach programmes for annual SMI checks that address the barriers people living with SMI face in accessing services

About us

The Dartford, Gravesham and Swanley area is located in North Kent, only a 15 minute commute by train from London St Pancras, and our main GP Federation office is metres away from Ebbsfleet International Rail Station. The area is undergoing considerable housing, retail and leisure development including the Ebbsfleet Garden City Area. The area also boasts the Bluewater retail centre, and there are a good variety of high performing schools including state, grammar and independent schools. All practices in the area are members of the GP Federation and we work together to design and deliver clinically led and managerially enabled services.

Main Duties and Responsibilities

Completion of physical health checks on patients with a diagnosis of severe mental health. The physical health check includes weight, blood pressure, blood lipid, blood glucose, alcohol consumption and smoking status.

  • You will be making contact with patients both on the telephone and face to face, so effective communication skills and professionalism are essential.
  • You will be signposting and cross-referring to other support services where applicable.
  • To deliver at point of care advice around improving health and providing easy to understand analysis of the outputs from the health check.
  • Requesting feedback from the patients as appropriate.
  • All health check data will be completed online, full training on the software will be delivered, however basic computer skills be required. The output from this will feed into the patents primary care Emis record.
  • Where the physical health check is known to be completed by another agency the results will be entered into the patients Emis record and coded appropriately.
  • It is important that you make sure data is inputted rigorously and consistently to guarantee that accurate records are kept up to date in compliance with Connect Healthcare's procedures and information compliance.
  • Governance and data protection is essential in relation to patients information. It is essential that data is kept private and secure at all times. This is something you will need to consistently adhere to.

The role is ideally suited to people with strong communication skills and those who are really passionate about making a difference to peoples lives on a daily basis. It also includes working with vulnerable adults and such the successful candidate will be required to apply for a DBS prior to starting the role
