Job responsibilities

The post holder will be an experienced clinician, who, acting within their professional boundaries will provide clinical assessment and management of patients within practice.

The post holder will support the delivery of high-quality care and will undertake service development and improvement of the local unscheduled care system, working within the team, across practices and with partners. Committed to the development of innovative practice and expanding evidence-based care which will contribute to the enhancement of the paramedic role.

You will be expected to provide and maintain a high standard of care for patients as well as providing support to the GPs and other members of the primary health care team. Supported by a skill mixed team, they will work within the practice, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for patients including assessment, diagnosis and treatment, to deliver quality patient services.

The following sets out the key role responsibilities for advanced paramedic practitioners:

• They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.
• They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

They will be able to:

• perform specialist health checks and reviews;
• perform and interpret ECGs;
• perform investigatory procedures as required, and;
• undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
• perform investigatory procedures needed by patients and those requested by the GPs
• They will support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
• The will communicate at all levels across organisations ensuring that an effective, patient centred service is delivered
• They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
• They will maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
• Prescribe/issue medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways
• Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.

Where involved in delegation of tasks, delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.

Ensure clear understanding and utilisation of referral mechanisms within the practice.

Prioritise own workload and ensure effective time-management strategies are embedded in own practice.

Maintain own CPD and contribute to own personal development by participating in annual appraisal with line manager, developing a PDP, and actively participating in agreed learning activities and evaluating effectiveness of learning in relation to role.

Regularly review own practices and makes changes in accordance with current and/or best practice, makes suggestions for improved practice and identifies where other activities affect own practice.

To achieve and demonstrate agreed standards of personal and professional development within agreed timescales.

To contribute positively to the effectiveness and efficiency of the service in which he/she works.

Essential

• 3 years minimum paramedic experience

Desirable

• Experience of working in Primary Care

Qualifications

Essential

• Recognised qualification in paramedic science (Foundation Degree or Undergraduate Degree)

Desirable

• Enhanced Clinical Assessment course
• Masters degree Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11.
• Or working towards Enhanced Clinical Assessment course or equivalent module Minor Injuries and illness module Prescribing qualification

• Must hold clean UK driving license
• Professional registration HCPC
