PCN Health & Wellbeing Coach (Therapy Support)

Job summary

This is an exciting opportunity for a Health & Wellbeing Coach (Therapy Support) to join South Lincoln Healthcare PCN as part of our Living Well Team.

The post holder will work with our existing Living Well Team across our practices (Bassingham Surgery, Branston & Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, Cliff Villages Medical Practice, The Heath Surgery, Washingborough Family Practice).

The role will involve a close working relationship with the Occupational Therapists within our Living Well Team.

Main duties of the job

The post holder will be an integral part of the PCNs Living Well Team, working alongside Occupational Therapists, Social Prescribing Link Workers and Care Coordinators to provide an all-encompassing approach to personalised care across the PCN.

The role will support people to develop their knowledge, skills and confidence in managing their health and care, acting within their professional boundaries to enable people to take pro-active steps to improve their health outcomes and quality of life, based on what matters to them.

The post holder will have access to ongoing supervision, skills development, and support so they are able to further build their skills and experience within the role.

The role will require the ability to work flexibly to meet the needs of the organisation and this may include working outside of core General Practice hours from time to time.

The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

Please note that the Health and Wellbeing Coach role is a non-clinical role.

Job responsibilities

Develops, manages, and maintains positive, appropriate relationships with patients, families and carers, demonstrating respect, kindness, compassion and empathy at all times.

Utilises coaching and motivation techniques to help patients identify their needs, set goals and support patients to achieve their personalised health and care plan objectives.

Provides interventions such as self-management education and peer support. Ensuring patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.

Provides education and expertise, supporting PCN staff to improve their skills and understanding of personalised care, behavioural approaches, shared decision making and ensuring consistency in the follow up of peoples goals with MDT input.

Works within the living well team to support people identified by the occupational therapists to achieve their personal goals, and maintain/regain independence to carry out everyday activities that matters most to them.

Provides feedback on patient progress and condition, recognises the impact of support and interventions, and suggests ideas for improvement when developing, reviewing and evaluating care plans based on objective evidence and experience.

May work alone within scope of role, for example in the community, and have their own caseload of patients with non-complex needs, as directed by the clinical team.

Maintains a working knowledge of local support available for patients and signposts people effectively in liaison with multi-disciplinary team colleagues

Ensures safe and effective use of prescribed equipment by patients, carers and family members, which may include fitting, demonstration, and teaching of safe and appropriate use. Ensures equipment is safe

Communicates in a way that is understandable and accessible, addressing any barriers to understanding. This can include the use of interpersonal, written and verbal and non-verbal skills. Exchanges information with patient and families using active listening, compassion, empathy, and coproduction techniques with the service user

Is responsible for the day-to-day planning of personal workloads

Follows organisational policies, protocols, procedures, and standards.

Reports any incidents, accidents or complaints in a timely manner, taking appropriate action where necessary

Liaises with the practices across the PCN

Develops themselves and the role through participation in clinical supervision, training and service redesign activities

Contributes to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety

Maintains accurate clinical records of all patient consultations and related work

Supports in the delivery of enhanced services and other service requirements on behalf of the PCN

Participates in the management of patient complaints when requested to do so and participates in the identification of any necessary learning brought about through clinical incidents and near-miss events

Undertakes all mandatory training and induction programmes
