Main Duties and Responsibilities:

Clinical:

Work with the PCCU team in identifying children and young people who would benefit from palliative care and hospice input and facilitate timely and sensitive conversations about their goals of care, including parallel planning, with their families and professionals involved as appropriate. This will involve:

Attending ward rounds and relevant meetings as required

Holistic needs assessment of patients on PCCU

Liaising with the paediatric palliative care (PPC) team and/or children's hospice team as appropriate.

Being a resource for information and advice on available palliative care service to the PCCU and wider MDT teams.

Attend weekly PPC MDT meeting and be involved in discussion of the relevant patients on PCCU and feedback to the PCCU team as necessary.

Where appropriate, support PCCU and PPC teams in facilitating the MDT meeting to discuss a patient care where palliative care or parallel planning is an essential part of the overall care.

Facilitate transfers to the hospice (or other care setting) for end-of-life care from PCCU or other acute setting, in partnership with the PPC team. This includes arranging familys pre-transfer visit to the hospice where appropriate.

Work collaboratively with the hospital clinical team, specialist nurses and hospice team to deliver a safe and effective service to the children with palliative care needs which makes best use of available resources, and which provides holistic, family centred support.

In partnership with the Ty Hafan Family Support Team and appropriate Health Board Teams as required, provide emotional, psychological and spiritual advice and support to families ensuring religious and cultural requirements are understood and supported.

Act as a source of information and advice on palliative and hospice care when decisions around pathways of care is being made for a critically unwell child in acute clinical settings across the geographical area.

In liaison with the PPC Consultant/Health Board PPC Specialist Nurse, to support with symptom management and palliative care of critically unwell child in acute clinical settings across the geographical area.

Support the discharge of children and young people with palliative care needs from PCCU to hospital wards, home or Ty Hafan including ongoing communication with the family and professionals post-discharge in partnership with the PPC CNS as needed. Education, training and service quality improvement:

Work with the practice educators in PCCU, Ty Hafan and other acute care settings to provide education to staff in Ty Hafan and Health Boards across the geographical area in relation to palliative care, supporting with the coordination and delivery of training/study sessions.

Maintain own professional portfolio in accordance with NMC revalidation requirements. Undertake relevant learning, training and development appropriate to your role to support professional development and maintain the necessary skills and competencies to meet the requirements of the role.

Adhere to the NMC Code of professional Conduct working within and accepting responsibility for maintaining agreed levels of competence.

Remain up to date with current literature and research within palliative care nursing to maintain your own professional and personal development in accordance with NMC revalidation requirements, attending mandatory training and study sessions as required.

Contribute effectively to the Performance Development Review process and 1- 1s.

Attend regular clinical supervision to encourage, develop and maintain reflective practice and receive professional support.

Maintain high standards of evidence-based care delivery to children and young people and ensure that standards of care are based where available on valid, current research findings.

Support coordination and delivery of debrief sessions for staff in partnership with colleagues within PCCU, PPC team, Ty Hafan and other Health Boards where needed.

Review and ensure pathways are in place and appropriate for each family, linking with existing bereavement support services within the Health Board, Ty Hafan and the wider community.

Contribute to the service quality improvement activities on PCCU and for the paediatric palliative care network by being proactive in the monitoring, reviewing and evaluating the service through agreed standards of care, audit, and other quality procedures, ensuring the voices of the patients and families also inform service development and improvement.

Contribute to the development of local and national palliative and supportive care initiatives, directives and strategies through involvement in consultation and feedback processes.

Participate in the development of new policies, standards and guidelines to improve the post holders own and others practice, including other professions. Communication:

Provide families with relevant and accessible written information regarding palliative care and hospice services.

Develop and maintain effective, collaborative, professional relationships and communications with, and liaison between, children, young people, families, professionals and non-professional carers. Establish and maintain a support system across professional, organisational and geographical boundaries. Work closely with the network and hospice to support exemplary multiagency working at the end of life.

Using highly developed communication skills, manage challenging situations that may arise e.g. strong disagreements in views, wishes and needs, complicated grief, professional ethical dilemmas.

Maintain accurate, clear and contemporaneous records and documentation in line with local policy and in accordance with the NMC Code of Conduct. Equality, Diversity and Inclusion

• 21. Post registration Diploma in
