Ty Hafan is a beautiful children's hospice located on the picturesque Vale of Glamorgan coastline. Covering the South, East and West Wales Ty Hafan offers a beautiful, newly refurbished working environment set in idyllic grounds a short distance from the metropolitan area of Cardiff.
We have an exciting opportunity for a qualified Paediatric Nurse to join our established Nursing team as a Clinical Nurse Specialist.

Main duties of the job
The role will be to work as part of the Care Services team working flexibly across the service to provide safe, effective and compassionate paediatric palliative care.
Youll be responsible for clinical assessments, developing comprehensive care plans, and liaising closely with internal and external partners to deliver seamless, high-quality care. This is a varied and rewarding role where youll use your specialist expertise to support children, young people, and their families, whilst working collaboratively with our wider multidisciplinary team.
The post is initially a 2 year fixed term contract with continuation beyond this dependent on the impact of the role and securing funds. The role is full time (37.5 hours) and will be primarily Monday to Friday but a degree of flexibility to meet family needs will be required and there will also be some out of hours on call duties.

Job responsibilities
Work as part of the paediatric palliative care network to develop safe care pathways between
Health Board services and Ty Hafan.
Work with the Head of Nursing and the Head of Community at the hospice to develop robust
referral pathways.
Liaise with Health Board and Local Authority colleagues to review and advise on pathways for
referrals.
Work closely with external partners when receiving referrals for a child or young person who
may require Paediatric Palliative Care services.
Present all referrals to Ty Hafan Referral Panel, providing all relevant information to support
decision making re acceptance for service provision as set out in the criteria for the service.
Receive emergency referrals to Ty Hafan and work with relevant members of the
multidisciplinary team to complete clinical assessments and advise the senior management
team when out-of-panel acceptance is necessary.
Undertake the initial clinical assessment prior to the childs first stay at Ty Hafan and work in
association with the Family Support Practitioner to developing a comprehensive holistic plan
of care.
Where necessary ensure the timely emergency transfer of a child or young person from
community or hospital setting, ensuring that all symptom management and clinical care
plans are in place prior to the admission to the hospice for care.
Provide expert advice and support to care services staff during a child or young persons stay
to ensure Paediatric Palliative Care is evidence based, delivered in accordance with care
plans and of a consistently high standard.
Work as an integral part of the Paediatric Palliative Care Team, ensuring attendance during
ward rounds at University Hospital of Wales and Ty Hafan.
Attending MDT meetings within the hospice, and the PPC meeting at UHW.
Develop a process for reviewing care plans in conjunction with the community nurse, the
community teams, the Hospice and with the family and external partners to ensure safe and
seamless care between hospice and home.
Support care services in the implementation of care plans for children and young people,
ensuring all changes are identified and communicated to all parties involved, including
external agencies.
Take part in quality improvement projects, championing best practice, and sustainable
change.
Partake in clinical audits, identifying areas requiring improvement, and working with the
quality improvement group to ensure required actions are embedded in practice.
