JOB PURPOSE

Appointed Registered Nurses will work as part of multi-disciplinary team, to deliver high-quality family centred care in our warm hospice environment. Post holders will additional provide direct clinical care to children and young persons in the receipt of our services, meeting their complex, chronic and palliative care needs.

MAIN DUTIES AND RESPONSIBILITIES

Key Tasks

To provide safe and evidence based clinical care to children and young persons resident in the hospice, and to support the continuity of this care in the wider community when necessary.

To advocate for participation of the child and their family in all aspects of care delivery, in collaboration with the multi-disciplinary team ensure that the total needs of child and family are met.

To ensure care delivered is in line with current regulations and guidance provided through professional bodies.

To support the assessment, planning and delivery of individual nursing care.

To administer medication and clinical interventions in line with Tŷ Hafan policy, NICE guidelines and NMC requirements.

To recognise and respond appropriately to clinical deterioration in children and young people.

To participate in standard setting, audit, data collection and quality review.

To utilise equipment and resources in a cost-effective way, in line with the concept of prudent healthcare.

To communicate effectively and in a timely and most appropriate manner with colleagues, children and families.

To maintain accurate, clear and contemporaneous records and documentation in line with NMC requirements.

To engage in other duties as appropriate to the band and level of responsibility of this role. Whilst consider own level of confidence and competence, ensuring delegated tasks are within own level of experience and training.

To maintain and extend clinical skills to meet the needs of the children in our care, through undertaking relevant training and competency assessment as appropriate to your role, in support of continued professional development.

To maintain and engage in Mandatory training, ensuring compliance remains within the agreed levels, adhering to organisation requirements enabling confident and competent practice.

To ensure you comply with NMC revalidation and annual re-registration requirements.

To share knowledge, experience and skills to benefit of others including colleagues and students. Supporting in the training of others as appropriate in line with the NMC code of practice.

To provide support and appropriate development opportunities to Volunteers, ensuring their skills and attributes are utilised to their full potential, whilst encouraging engagement and fulfilment in accordance with Tŷ Hafan Volunteer Policy and Procedure.

Health, Safety and Wellbeing:

To take reasonable care for the health, wellbeing and safety of self and others, who may be affected by actions or nonactions in the workplace.

To abide to Tŷ Hafans Infection Prevention and Control (IPC) policies, undertaking the necessary level of training and adhering to current IPC guidelines in affect at Tŷ Hafan hospice.

To adhere to the process of reporting accidents and incidents, in support of investigation, auditing and possible improvements to practice as required.

All those who are in contact with children and young people have a responsibility to safeguard and promote their welfare and should know what to do if they have concerns about safeguarding/child protection issues.

Person Specification

Experience

Essential

• Experience of working with children with complex and specialists needs.

Desirable

• Previous clinical experience of working in a similar nursing environment.
• Experience working with children who require non-invasive ventilation support.
• Clinical leadership- Dependant on experience candidates may be expected to supervise junior staff and take charge of shifts.
• Experience of clinical audit.
• Previous experience of using electronic patient records and care databases.

Qualifications

Essential

• Current NMC registration.
• Nursing degree or diploma with evidence of continuous professional development.
• Registered Nurse (Child, learning disabilities or adult/general nurses with recent practice within a paediatric setting).

Desirable

• Completion of a post-graduate nurse foundation program and or preceptorship program.
• Practice supervision and/or Practice assessor training (previously known as mentorship).
