We have an exciting opportunity for a Palliative/EOL Frailty Practice Development Sister/Charge Nurse to join our team at Trinity Hospice. This fabulous opportunity has been secured by support from a Hospice UK grant for a period of 2 years to support the delivery of outstanding quality care to frail and vulnerable people on The Blackpool, Fylde, and Wyre Coast.

As part of our commitment to a continuous improvement strategy for this group of individuals and, in order to reduce variation in care with the focus being on standardising practice we have created this innovative and autonomous post. The Palliative/EOL Frailty Practice Development Sister/Charge Nurse role will be pivotal in achieving this aim by leading the way in education and support, ensuring all those who care for the frail and elderly are competent in developing evidence based, comprehensive assessment as part of personalised plans based on individuals circumstances.

Main duties of the job

This is a new role within Trinity Hospice and will be pivotal to the delivery of a new pathway and model of care for the frail and elderly. The successful candidate will ensure that the frail person is at the centre of decision making across all settings on the Blackpool Fylde and Wyre Coast ensuring that the risk of these patients being admitted to hospital unnecessarily is reduced. A further aim of the role is to work towards a model that ensures only those patients who actually require hospitalisation are admitted and discharge is proactively expediated to their PPC or PPD.

The successful candidate will be expected to work autonomously supported by the senior nursing team within Trinity which consists of Palliative Care Consultants, Medical Director, Director of Clinical Services, Assistant Director of Clinical Services, Head of Community, Hospice at Home, and hospital services along with 5 x qualified Advance Clinical Practitioners.

The role will require a collaborative approach working alongside our partners on the Fylde Coast who care for the frail and elderly including nursing homes, care homes, district nurses, GPs, and the team at Blackpool Teaching Hospital particularly our palliative care nurses within the ED department.

A key working relationship will be the Frailty Team and working alongside them to identify the time when the palliative role will make the most impact on preventing hospital admission.

Trinity is the only hospice serving the Fylde Coast and, with a proactive and collaborative approach to provide the highest standard of palliative care to the diverse population it serves. Our services include an eighteen bedded adult inpatient unit, a four bedded childrens unit, a 365-day community team with a separate night hospice @ home team, a team of hospital palliative care nurses, a Lymphoedema clinic, bereavement support, carer, and patient psychological and counselling support as well as our brand-new Living Well Service.

You will be responsible for providing specialist nursing advice and support to professionals working with the frail and elderly, whilst also supporting patients and their families to avoid unnecessary and very often distressing hospital admissions. You will work closely with our Physiotherapist who also has his qualification in Advanced Clinical Practice ensuring those who can benefit from the Living Well Services many rehabilitative and supportive sessions are referred in.

You will be expected to maintain clinical credibility by acting as a clinical practitioner within your own area of expertise.

You will contribute to, and influence the development, evaluation, and audit of this frailty project by working collaboratively with General Practitioners s, primary health care practitioners, statutory and voluntary agencies influencing the delivery of evidence-based care for the patients and families caring for the frail and elderly.

You will lead on, and offer expertise and guidance to professional colleagues, families the general public and others in supporting the best outcome for the frail and elderly.

You will be expected to take advantage of the clinical supervision and practice development available at Trinity and work alongside the Clinical Educator and Practice Facilitator to provide any necessary specialist training.

You will be involved in quality improvements that will promote the health and wellbeing of those who are frail and elderly and their families. In line with health and social care policy, guidelines, and legislation.

You will need to work collaboratively within the PCNs to influence practice relating to the support and treatment of the frail and elderly replicating good practice in those areas that are achieving the best outcomes for this group of patients.

Good practice will need to be cascaded to those areas that are evidenced to have higher than average admission to hospital data whilst ensuring the staff are supported to feel confident with the support they can offer.

A key area of further development will be to achieve the Fylde Coast Strategic Aim that all patients with a palliative diagnosis will have an individual advanced plan of care developed in a caring and supportive way including the appropriate family members as required. This will involve developing Advance Care Planning Clinics to support professionals with the knowledge, skills, and confidence to be able to support those frail and elderly patients develop a plan of care that meets their needs including the limitations of treatment but also the goals, spiritual needs, and desires of the individual.

You will be expected to demonstrate excellent leadership skills, working as an autonomous practitioner supported by the Senior Clinical Team at Trinity to influence change of practice in different settings in relation to care of the frail and elderly. This will be made possible by providing the education, knowledge, and skills to enable high quality, safe care for the frail and elderly in the most appropriate environment.

You will assist in the development of practice, clinical protocols and policies for the clinical team Identifying, monitoring, and reviewing deficiencies in procedures and implementing remedial action.

You will be required to facilitate the use of up-to-date information technology in order to collate accurate and timely information as and when required.

You will have the skills to encourage and motivate others to be forward thinking in ideas that address the needs of the frail and elderly that will be innovative whilst managing and developing new ways of working, e.g., patient led advance care plan clinics, champions within each area etc.
