We have a fantastic opportunity to join our friendly and welcoming team at Rugby Myton Support Hub.
The Hub supports patients with life limiting illnesses and their families and carers, through a programme of activities, support groups and information sharing. It's a fun-loving environment which also respects the need for sensitivity.
This is a part time position with 30 hours per week, from Monday to Thursday.

Main duties of the job

About you
You could be our Hub Coordinator if you have:
- NVQ Level 3 in Health and Social Care, or equivalent
- Experience of working in a community setting, palliative or end of life care, or a day centre
- Excellent communication and interpersonal skills
- Experience of coordinating a programme of events
You might have experience as a Day Centre Coordinator, Events Coordinator or Activities Coordinator in a similar setting.
You will have excellent observational skills and be able to communicate clearly with a wide range of people.
As a friendly and approachable individual, you will appreciate the value in giving someone with a palliative illness wellbeing social support to enhance quality of life.

Role purpose:
The Rugby Myton Support Hub Coordinator is responsible for providing practical information and support to individuals, their families/carers to help reduce the fear and uncertainty of living with a palliative life limiting condition. The Coordinator will develop, organise and evaluate stimulating programmes of activities, support groups and information events to enhance the lives and wellbeing of people with palliative and end of life care needs, their families/carers, in the Rugby area.
The Hub Coordinator where appropriate will also make referrals with the individuals consent to the Hospices Patient and Carer Wellbeing Service to ensure individuals who require specialist support, have access to planned outpatient appointments.
The Coordinator will work under the supervision of the Patient and Carer Wellbeing Service Lead to ensure the Hub is accessible and welcoming for all those who need it.
The Hub programme will be monitored and evaluated to ensure it meets the needs of the local community. As the Support Hub is not a clinical service people attending the Hub will continue to receive clinical support from their GP, District Nursing Team, Medical Team and any other healthcare professional involved in their care.
Main Duties & Responsibilities
- Be the first point of contact, signposting, providing information and support for visitors and provide a warm and welcoming environment.
- A key aspect of the role involves face-to-face and telephone communication with patients, relatives, carers enabling them to express their emotions and needs in a supportive environment. Communication in this context requires a high degree of empathy, sensitivity, understanding, diplomacy, honesty and integrity.
- To co-ordinate and provide access to the right information and resources to support people in making decisions about aspects of their own care, enable independence and support self-management as appropriate.
- Build and maintain professional relationships with partner providers including primary and secondary health care teams, voluntary organisations and local authorities.
- To connect with all key organisations to explore opportunities of who would make invaluable contributions to the Support Hub programme of offer.
- To promote the Support hub in the community and encourage the use of the space for groups to meet and hold event.
- Ensure individuals, their families /carers receive access to information considering social, spiritual, cultural and ethnic needs.
- Monitor and maintain support and information noticeboards within the hub setting. Identifying new sources of patient/carer information of local services and national initiatives.
- To lead with the group sessions where appropriate and required and to direct the volunteers to assist in the organisation of the sessions/activities.
- To contribute to the development of new ideas and innovation to keep the support Hub up to date and responsive to the local community needs.
- To contribute to the marketing and communication plan of promoting the Support Hub to the public who could benefit in accessing the service provision.
- To oversee the day to day running of the Support Hub ensuring adherence to procedures.
- Perform required administrative duties and maintain accurate record keeping to ensure there is robust activity data reporting.
- To regularly assess the effectiveness of the support Hub offer.
- Oversee the rota of the volunteers to ensure there is adequate cover for the safe running of the Hub with support from the PCWBS Lead.
