Job summary

St Gemmas Hospice is seeking a Community Clinical Nurse Specialist (CNS)who is passionate about delivering holistic care and supporting patients in a community setting.This is an exciting opportunity to join a dynamic team and have the opportunity to supportcurrent service developments.

  • Significant experience at Band 5 working in specialist palliative care or caring for palliative care patients in a community or hospital setting
  • Experience and knowledge of symptom management
  • Ability to communicate effectively in complex clinical situations and with colleagues
  • Desire to be part of a flexible and ambitious multi-disciplinary team
  • A driving licence and access to a car for work related purposes

St Gemmas Hospice is a Leeds-based charity providing expert care and support for local people with life-limiting and terminal illnesses. We offer care to patients and their carers in the Hospices In-Patient Unit, through the St Gemmas community team, and via our day services, which include a Day Hospice and Out-Patient Department.   • First level Registered Nurse, Degree in Nursing or related subject (or willing to work towards), Advanced communication skills training (or willing to work towards), Non-Medical Prescribing Qualification (or willing to work towards), Recognised mentorship/educator/assessor qualification (or willing to work towards)

Desirable

  • Post registration education in Palliative Care

  • Post-registration experience in a variety of settings, Significant experience at Band 5 in specialist palliative care or caring for palliative care patients in a community or hospital setting, Experience and knowledge of symptom management, Experience managing a caseload, Teaching experience, Demonstrates an understanding of current issues and best practice in palliative care, Experience of communicating regarding complex, sensitive issues with patients, carers and professionals, Understanding of loss and grief and its impact on individuals and families, Knowledge of advance care planning
