Main duties of the job

We are looking for an experienced Clinical Support Nurse to join our team at St Johns Hospice. The St Johns Hospice@Home service provides specialist support by placing Health Care Assistants experienced in community work and trained in palliative care with patients in their own homes up to 24 hours a day in order to support them to be cared for and die in their preferred place of care.

The Hospice consists of 15 bedded Inpatient unit, a Day Care Unit and a Community Specialist Palliative Care team maintaining highest standards of palliative care within a multidisciplinary environment. Services are provided for people with cancer and other life-limiting illnesses and for their significant others.

Located in leafy St Johns Wood on the Jubilee line, minutes from Central London, our independent private Hospital has just been rated the number one in the country for care in the latest Bupa survey. Uniquely, all profits fund our onsite Hospice, St Johns.

Job responsibilities

  • To maintain confidentiality surrounding patients on the Hospice at home caseload
  • To ensure documentation is completed accurately and within agreed timescales using Hospital approved documentation, in line with NMC and Hospital guidelines
  • To demonstrate a professional approach to work, and act in accordance with the Hospital Code of Conduct and the NMC Code of Professional Conduct at all times
  • To participate in staff appraisal, staff development and in service training activities
  • Receive H@H referrals, assess whether criteria(s) are met, and accept or decline for H@H care
  • Receive patient handovers and updates from H@H carers as appropriate
  • Organise and co-ordinate H@H bookings assisted by the H@H administrator
  • Complete and circulate weekend Hospice at home handover, give clinical handovers to H@H carers
  • Ensure effective liaison with patients, families, carers, members of the primary and secondary care team, hospice multi-professional team and other outside agencies especially related to the ongoing organisation of care packages or discharge from H@H service
  • Ensure patient clinical records are maintained using the Cross care data base
  • Participate in multi-professional MDT meetings
  • Undertake home visits to patients receiving the service where necessary
  • Provide positive leadership, a caring supportive environment and an excellent role model for the H@H Carers
  • In conjunction with the H@H Coordinator support the professional development of the H@H carers
  • Work closely with other Hospice team leaders and other members of the Hospice Management team
  • Take responsibility for day to day coordination of the H@H service
  • With the H@H Coordinator take part in recruitment and selection of Health care assistants for the H@H service
  • Support H@H coordinator with HCA appraisals
  • Ensure compliance with Lone worker Policy
  • To assist the H@H Coordinator with any teaching and educational programmes in the Hospital/Hospice
  • Assist the H@H Coordinator with the H@H induction training programme for newly appointed H@H carers
