Main duties of the job

The Deputy Nurse Manager at Nimbuscare, will be part of this innovative service, working within the Community Frailty Hub, supporting the Nurse Manager in overseeing the clinical and operational delivery of services across the Nursing and Health Support Worker (HSW) teams as part of the Frailty Prevention function of the Community Frailty Hub. The role provides delegated leadership, supervision, and coordination to ensure safe, effective, and high-quality patient care.

The postholder will contribute to the day-to-day management, assist in driving quality improvements, and promote best practice in frailty prevention and community-based care. Working as part of a multidisciplinary team, the Deputy Nurse Manager will help ensure that service delivery aligns with organisational, clinical, and regulatory standards while supporting the integration of new services, pathways, and digital innovations.

Frailty prevention - Support frail people to remain independent for longer by providing evidence-based Comprehensive Geriatrics Assessments (CGAs) in the community responding cohesively alongside the wider MDT.

Integrated neighbourhood working - Regular attendance at the Integrated Neighbourhood team MDT. When required supporting the smooth running of these meetings as Chair.

Clinical Leadership & Frailty Care

  • Work cohesively with the wider MDT, including Palliative and EOL care teams in the community, to coordinate holistic, person-centred care and support smooth transitions across pathways
  • Assist in monitoring and auditing clinical practice to ensure adherence to patient safety, and quality standards.

Team Leadership & Line Management
