We are looking for a dynamic physiotherapist to join our team. You will have excellent communication and prioritisation skills with a flexible person-centred approach to join our supportive Community Frailty Team.

Looking for a role where you will be working with some of the city's most vulnerable people. With the Frailty team your role will primarily be providing preventative assessments to support the team in preventing progression of frailty and unnecessary admissions to hospital. Providing short term interventions that will support maintaining safety and independence at home. You will also have the opportunity to work in other areas including expediting early discharges from hospital.
You will be working as part of an expanding Frailty HUB which has anticipatory, crisis and discharge arms. We integrate with other services, both primary and secondary care, social services, CVS and YAS.

Main duties of the job
To complete physiotherapy assessments to in the community preventing admission to hospital and providing community rehabilitation. This will include treatment planning, assessing risk, promoting, and enabling independent functioning.
To provide training, supervision and advice to therapy assistants, and other therapists/ nurses in the team
To participate in the planning, development, and evaluation of community therapy service, holding responsibility for defined projects.
To maintain close working relationships with other health professionals /referrers to ensure efficient, effective, and appropriate delivery of the service.
A full driving licence and access to a vehicle that can be used for work is essential

To maintain robust working relationships with the wider MDT including social care and voluntary sector colleagues.

Take responsibility for organising, triaging, and prioritising own caseload in line with service and patient needs. Adapt and readjust plans as situations change or arise, ensuring timely and effective interventions. Motivate and encourage patients to participate in treatment programmes and where

To use a range of verbal and non-verbal communication tools to communicate effectively
with patients and relevant others. The caseload will include individuals who have difficulties
in understanding or communicating.

To undertake the measurement and evaluation of your work and current practices through
