Intermediate Clinical Practitioner - Urgent Care Response

Job summary

We are recruiting for an intermediate clinical practitioner to work as part of the Urgent Care Response Team within Community Services.

Working in the UCR team you will respond to the Frailty GP's request for home visits, in a timely manner and within a 2-hour response time and provide clinical assessment of the patient in their own home, following this you will debrief to the Frailty hub GP's or Nurse consultant who will support you to make a differential diagnosis, management and treatment plan.

We offer a progression/ development process and the opportunity to work alongside a dedicated team of general practitioners, nurse consultant and urgent care practitioners. The team is supported by trainee advanced clinical practitioners from a variety of backgrounds including adult nursing, mental health nursing, physiotherapy and paramedic science.

Main duties of the job

The post holder will demonstrate the knowledge and skills of an intermediate clinical practitioner and will:

With delegated direct or indirect support complete clinical assessment and care for patients.

Under supervision will complete complex frailty, physical and mental health assessments and form differential diagnoses and appropriate treatment.

With guidance from enhanced/advanced or general practitioners will prescribe medication within scope of practice as appropriate (however this is not currently a requirement for the role) select and order appropriate investigations independently.

Help to Identify the need for improvement and change within the area of responsibility and following agreement and approval, support with the implementation and development of new policies, procedures and practices and where appropriate with support implement service re-design solutions to better meet the needs of the patients and the service.

Travel or working across or at alternative sites for both clinical and supervisory duties may be required.

Job responsibilities

The post holder will be working under direct or indirect supervision as a trainee enhanced or intermediate clinical practitioner in the delivery of high-quality clinical care delegating clinical decisions to enhanced/advanced practitioners or the general practitioner team.

The post holder will start to build a portfolio of evidence within clinical practice and support, nursing and allied health teams to improve the patient experience, supporting the trainee and advanced clinical practitioner, service lead and medical and nursing team with day-to-day clinical activity within the Frailty team. There may be a requirement to provide cross site or alternative site cover dependent on operational need.

  • Will support and on delegated direction completion of ReSPECT forms and advanced care planning, tailored to the patients level of frailty, clinical condition, co-morbidities and wishes.

Ensure that conversations with the patient and family members are properly recorded and discussed as appropriate.

Clinical Responsibilities

  • With support as required perform clinical assessment and feed back to the advanced clinical practitioner or general practitioner to enable them to form a differential diagnose and plan.
  • Support with the implementation of the plan, and evaluate individual treatment plans, interventions, and care packages for patients, ensuring safe and appropriate management, timely referrals, and safety netting as required.
  • Collaborate with the multidisciplinary team (MDT) to expedite safe and timely triage consultations, interventions, and reviews.
  • If appropriate and competent prescribe within scope of practice with support as required, review medication for therapeutic effectiveness, and ensure treatment is tailored to patient needs, adhering to contraindications, drug interactions, evidence-based practice, national and practice. protocols, and within scope of practice to optimise outcomes.
