Salary Scale: Based on experience and stage of practice development:
- Stage 1 (Entry Level): £38,885
- Stage 2 (Intermediate/Developing): £42,854.70
- Stage 3 (Advanced/Experienced): £46,825
Contract: Fixed Term Hours: 37.5 hours per week (2 posts available) Base Location: York Frailty Hub, Gateway 1, Holgate Park Drive, York, YO26 4GG Travel across sites and within the community required in line with service needs
About the Role:
We are excited to be expanding our Urgent Community Response (UCR) service within the Community Frailty Hub in York. Were looking for two Intermediate Clinical Practitioners to join our dynamic and supportive multidisciplinary team.
This is a fantastic opportunity for motivated and compassionate clinicians looking to develop their skills in urgent care, frailty management, and community-based care. You will play a key role in providing rapid response to patients with frailty-related health concerns, helping to avoid unnecessary hospital admissions by delivering timely, person-centred care in the patients usual place of residence.
Main duties of the job
Key Responsibilities:
- Deliver urgent two-hour response assessments and interventions for people living with frailty in their own homes.
- Support care delivery under the supervision of advanced clinical practitioners or GPs.
- Work as part of a wider multidisciplinary team (MDT) including GPs, nurses, therapists, and social care professionals.
- Maintain a clinical portfolio and contribute to service improvement.
- Work flexibly across a 7-day service (8am8pm) in line with rota requirements, including some weekends.
