We are seeking a dedicated Qualified Nursing Associate to support the assessment, planning and delivery of care for patients living with frailty under the supervision of a registered nurse.

Interviews are expected to be held on 23 July in York. Kindly take this into consideration when submitting your application.

Main duties of the job

Main Duties and Responsibilities:

Contribute to service delivery and patient care.

- Work closely with other healthcare professionals, including physicians, occupational therapists, physiotherapists, social prescribers and VCSE partners and social workers, to ensure a holistic approach to patient care.

- Attend multidisciplinary meetings to discuss patient progress and care plans.

- Advocate for frailty patients' needs and ensure effective communication among all involved parties.

Patient & Family Support:

- Provide support and education to patients and their families about frailty management, self-care, and available resources.

- Work in partnership with patients, families and carers to monitor the condition and health needs of the patient and escalate any concerns to nurse in charge.

- Address patient, family and carers concerns and ensure they are involved in care decisions where appropriate.

- Help patients and families navigate healthcare systems and community resources.

Compliance & Documentation:

- Maintain accurate, detailed, and up-to-date patient records in line with organisational policies.

Staff Training & Development:

- Attend training programs to enhance knowledge and skills related to frailty care.

- Promote evidence-based practice and ongoing professional development.
