Job summary

From 1st October 2023 Nimbuscare will be working in collaboration with Totally Plc to deliver the GP Out of Hours service in York, Malton and Scarborough. Applying for this role will enable you to work in all three locations.

You will have access to SystmOne, to ensure visibility of patient records and continuity of care. You'll be able to handover patient related tasks to their registered GP Practice and initiate referrals via the Nimbuscare operational services team. This aims to ensure a more integrated urgent care service for our patients.

We will support you to work as a team with colleagues across York, Malton and Scarborough overnight, by using SystmOne instant messenger for peer clinical support and to manage calls whilst you take a break.

If you have been away from out of hours for some time or are newly qualified, we will support you to join the team by offering shadowing opportunities and mentorship to help build your knowledge and experience.

You will be responsible for providing telephone triage/consultations, face to face consultations and home visits. You will ensure that patients are dealt with appropriately and efficiently, whilst maintaining a high-quality service.

You will report to the Urgent Care Lead at Nimbuscare and be fully supported by the team including the Senior Operational Manager and HR Manager.

Main duties of the job

  • You will undertake telephone triage, telephone consultations, face to face consultations and home visits.
  • Make professional, autonomous decisions in relation to presenting problems.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Manage patients appropriately including initiating investigations, making referrals to secondary care or to other providers as appropriate.
  • Provide health promotion
  • Record clear and contemporaneous computerised consultation notes to agreed standards.
  • Compile and issue computer-generated electronic prescriptions
  • Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).
  • Work with and support other members of the primary health care team in delivering high quality primary care.
  • Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health and local NHS trust.
