Would you like to be part of the new Urgent Care Delivery within the Community Setting in York?
If the answer is yes, we have an exciting opportunity for you to join York Integrated Community Team.
The role is offering an exciting opportunity to be part of the development of a new service model, offering urgent community response. This will:
- Enable people to live healthy independent lives for as long as possible in their own homes.
- Reduce the need for escalation of care to non-home settings
Main duties of the job
- YICT cover all 10 practices in York and you will be working closely with Primary Care to offer an urgent care response in the community
- You will be responsible for providing face-to-face visits to patients who have been triaged and deemed to require a 2-hour response
- You will act as an autonomous clinician and take responsibility for the assessment, treatment and/or to appropriately refer patients to specialist services when required
- Be compliant with the NMC or NCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance
- Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgment to identify the potential diagnosis
- Independently decide on and carry out treatment, including the prescribing of medicines
- Make onward referrals to other services as appropriate
- Ensure the provision of continuity of care including follow-up
- Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed
- Work independently, but as part of a local and wider health care team
- Provide leadership to the wider team and make sure that each patient's treatment and care is based on best practice
