Would you like to be part of the new Urgent Care Delivery within the Community Setting in York?

If the answer is yes, we have an exciting opportunity for you to join York Integrated Community Team.

The role is offering an exciting opportunity to be part of the development of a new service model, offering urgent community response. This will:

  • Enable people to live healthy independent lives for as long as possible in their own homes.
  • Reduce the need for escalation of care to non-home settings

Main duties of the job

  • YICT cover all 10 practices in York and you will be working closely with Primary Care to offer an urgent care response in the community
  • You will be responsible for providing face-to-face visits to patients who have been triaged and deemed to require a 2-hour response
  • You will act as an autonomous clinician and take responsibility for the assessment, treatment and/or to appropriately refer patients to specialist services when required
  • Be compliant with the NMC or NCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance
  • Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgment to identify the potential diagnosis
  • Independently decide on and carry out treatment, including the prescribing of medicines
  • Make onward referrals to other services as appropriate
  • Ensure the provision of continuity of care including follow-up
  • Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed
  • Work independently, but as part of a local and wider health care team
  • Provide leadership to the wider team and make sure that each patient's treatment and care is based on best practice

Accountable for the delivery of efficient, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council / Health and Care Professions Council CPC NICE, Local Authority Guidelines)

1. To effectively and efficiently manage Urgent 2-hour response" home visits

2. Required to work from any surgery site or any other health care settings (e.g. home visits, in the community, residential and nursing homes) as deemed necessary to the smooth running of the Practice

3. Act as an autonomous clinician and take responsibility for the assessment, treatment and/or to appropriately refer patients to specialist services when required:

4. To be compliant with the NMC or HCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance.

5. To effectively and efficiently, take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the differential diagnoses in a fast-paced environment

6. Independently decide on and carry out treatment as appropriate and within the scope of your competencies

7. Prescribing (if prescriber): Its the post holders' responsibility to adhere to any guidelines, protocols, and best practice.

8. Responsible for applying your extensive practice experience to plan and provide skilled and competent care to meet patients health and social care needs, involving other members of the wider health care team as appropriate.

9. Responsible for referring patients for investigations and/or the appropriate specialist where appropriate make a differential diagnosis.

Operate within an urgent care clinic and team environment:

1. To operate in a fast paced environment, providing effective, safe and efficient care to our patients and to provide an efficient service to colleagues in the urgent care team

2. Work independently, although often as part of an YICT team working closely alongside a GP / Advanced Nurse Practitioner / other clinicians.

3. Review and amendment of urgent care protocols as required

4. Advise management of equipment / maintenance requirements.

5. Contribute to QoF data collation as required

6. Record all Patient Contact on Clinical system in a timely manner

Urgent Care Practitioner Specific Accountabilities

  1. Take a comprehensive patient history and holistic assessment.
  2. Making professionally autonomous decisions, for which they are accountable
  3. Receiving patients with undifferentiated and undiagnosed problems and making an assessment of their health care needs, based on highly-developed knowledge and skills, including physical examination
  4. Screening patients for disease risk factors and early signs of illness
  5. Making differential diagnoses using decision-making and problem-solving skills
  6. Developing and providing the patient with an ongoing care plan for health, with skilled and competent care to meet the patients' health and social care needs involving other members of the team as appropriate. This should also have an emphasis on health education and preventative measures.
  7. Ensure the provision of continuity of care including follow up visits

8. Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed

9. Request necessary investigations, and providing treatment and care both individually, as part of a team, and through referral to other agencies.
