Job summary

Internally this role will be known as 'Patient and Family Support Services Manager'.

St Lukes Hospice is the major provider of specialist palliative care to adults in Plymouth and South West Devon. St Lukes clinical services consist of a community team, specialist inpatient unit, hospital team, End of Life Urgent Care Service and an education department. The Patient and Family Support Service endeavour to provide specialist support to patients and clinicians across all services. We are committed to ensuring that everyone who uses our hospice is treated with dignity, fairness and respect and is empowered to make choices about their lives.

We are looking for an experienced, compassionate, and innovative social worker who can lead a service providing support and advice to patients with progressive life-limiting illnesses. Our aim is to ensure consistent access to quality person centred social care and psychosocial support. The Patient and Family Support Service offer pre and post bereavement support to both adults and children.

The primary focus of the role, whilst also carrying a complex caseload, is to operationally manage a team including 3 social workers, a social care and bereavement support worker, a children and families support worker, counselling students and bereavement and befriending volunteers.

Main duties of the job

Key responsibilities

Lead on the development of in-house patient and family support services, facilitate signposting, make referrals and collaborate with external agencies.

Support the team in providing social work and psychosocial intervention in relation to issues such as emotional distress, family conflict, mental health, learning disability, substance misuse, safeguarding, Mental Capacity Assessments and Best Interest care planning including Deprivation of Liberty Orders.

Contributing to multidisciplinary team decision making and advocating for clients and families within St Lukes and externally.

Engage with exciting current or new projects such as joint ventures with other agencies. Working in partnership within and external to the organisation to continually improve and work together to provide a person-centred progressive service.

Our ideal candidate

Were looking for an experienced social worker who has the passion, skills, commitment, and resilience to lead this established team. To succeed in this role you will also need:

Registered social worker qualification

Current HCPC Registration

Youll hold a degree or diploma in social work or equivalent.

Significant post qualifying experience working with individuals and families in a multidisciplinary and/or health care environment

Track record in operational management

Strong interpersonal & communication skill

About us

The post holder will be based at our Turnchapel Site in Plymouth and the post holder will visit clients at home across South and West Devon. We are currently operating a hybrid working model where there will be the opportunity for homeworking.
