Job summary

The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within ELM Alliance, delivering care on behalf of the Primary Care Networks (PCNs) outside of core general practice hours.

The Nursing Associate will work in close partnership with the other key members of ELM Alliance, ELM Alliance Management Team and external organisations to implement & deliver the Enhanced Access on time and within budget.

Work with the qualified nurses to deliver individual treatment packages with an emphasis on collaborative care that focuses on recovery and the need to work towards discharge where appropriate. The post holder will work with other members of the Multi- disciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient.

Main duties of the job

The Nursing Associate will-

Be a member of the clinical team.

Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.

Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition, be able to identify anomalies that should be referred to a first level nurse or one of the medical staff

Refer patients to other clinicians as necessary

Provide routine clinical care to patients according to locally approved protocols, under supervision of the Nurse Practitioner.

Work within the hubs delivering the STAR service Extended hours on behalf of the PCNs

About us

ELM Alliance Ltd was formed by the GP Practices in 2017 and has approximately 250 people working for us and with us. We deliver NHS services to the patients of Redcar, Cleveland and Middlesbrough and are wholly owned by the local GP Practices. Our services include primary care general practice, services to care homes and community services such as dermatology. The landscape is changing; both the way we interact with patients and the local structures. It is an exciting and dynamic time to work with General Practice, other NHS providers and the voluntary sectors.

Specific Responsibilities

Understand the principles of healthy living and provide relevant health promotion literature to patients.

Assist clinicians in the emergency management of patients.

Collect patient data such as blood pressure, height, weight and ECG in accordance with service protocols and your competence to operate the necessary equipment.

Record data on patient electronic records in an accurate and timely manner.

Summarise patient notes and provide administrative assistance to the nursing team.

Provide smoking cessation support to patients in accordance with your training and protocols.

Run Blood Pressure and Cardiac Clinics if required.

Provide cervical screening in accordance with service protocol on patients who have been recalled.

Provide wound management to patients who have a care plan.

Prepare the consulting and treatment rooms for effective consultation, maintaining stocks of examination instruments, stores and specimen collection materials, and ensuring couches and work benches/drawers are kept clean.

Maintain the system for monitoring and ordering clinical supplies.

Ensure that all specimens taken by clinicians, or brought in by patients, are ready for collection by pathology service.

Assist in maintaining safe storage of drugs and vaccines under the direction of the on duty Nurse Practitioner.

Ensure that you comply with infection control guidelines.

Maintain patient privacy and dignity at all times, respecting patients rights, and preferences, and adopt a non-judgemental manner when interacting with patients.

General Responsibilities
