Job Summary
Deliver high quality, person-centred care under the direction of a Registered Nurse or Registered Nurse associate.
Outreach Heath Care Assistant to work under direction of outreach team
Work as a valuable part of the nursing and outreach team delivering care that focuses directly on the needs of the person
Carry out specific delegated tasks to a high standard and competency, under the direction and supervision of the Registered Nurse or Registered Nurse Associate and Outreach team
Clinical Responsibilities:
Work under the direction of the Registered Nurse or Registered Nurse associate to provide and deliver a high standard of nursing care
Work under direction of outreach team to provide high level of patient care
Assist in the provision of care to meet the physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers and refer to other professionals as appropriate.
Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
Maintain an understanding of caring for individuals with specific needs, for example dementia, mental illness, learning disabilities
Ensure the privacy, dignity and safety of people is always maintained
Report back and share information with the registered nurses and Registered Associate Nurses and Outreach team on the condition, behaviour, activity and responses of people and refer to other professionals as appropriate.
Ensure that accurate documentation and record keeping is completed that reflects the care delivered. This includes both electronic and all paper records. Documentation should ideally be completed immediately after the intervention or incident but before the end of the shift. Each Healthcare assistant should document the care and intervention that they have provided.
Following training and competency assessment as appropriate for role requirements, perform and record clinical observations including blood pressure, temperature, respirations, pulse, and any additional requirements within the speciality.
Working under the direction of a registered nurse or Registered nurse Associate and following training and competency assessment undertake venepuncture
Following training and competency assessment to develop clinical skills within specific role requirements; these may include for example -removal intravenous and subcutaneous cannula when no longer required, psychological support for patients and carers, complimentary therapy support working under direct supervision of Allied Health care professional and Registered nurse or Nurse Associate.
To monitor stock levels and supplies and ensure timely and appropriate ordering
To take reasonable care to avoid injury during course of work and co-operate with the hospice and others in meeting statutory regulations in accordance with the Health and Safety at work Act 1974
Act in accordance with all Hospice policies and procedures and within expected values and behaviours
Follow all information governance guidance and policies
Maintain confidentiality as outlined within data protection policies
Communication and Working relationships
Actively participate in and attend staff meetings
To attend mandatory training sessions and complete e-learning required for role.
To participate in annual appraisal scheme, maintain own personal continuing education and development by attending training as identified in appraisal.
Undertake further training as required, prior to carrying our any additional duties identified as relevant to role, to comply with Hospice policies and procedures.
Participate in the induction of new staff to their area and support less experienced workers in this role.
Safeguarding Duties
It is the responsibility of the post holder to be aware of and follow the legislation and guidance regarding Safeguarding Adults and Children as stated in the Hospice Safeguarding Policy. This applies to all staff regardless of which member of the family is the primary client. The post holder is responsible for ensuring they receive the appropriate level of safeguarding training according to their role.
