Job summary
Channel Health Alliance, a GP Federation providing NHS Services is seeking to recruit Bank paramedic practitioners and paramedics. The role is due to the delivery of the home visiting service to support primary care that Channel Health Alliance provides across all localities within the South Kent Coast area.
This post will work clinically seeing patients in their own place of residence on behalf of the GP practices who triage and identify patients who meet the service criteria. The post holder will ensure the smooth running, clinical quality and safety of the service.
The post holder will have the skill set to maintain high standards of holistic and comprehensive clinical assessment of patients with a variety of conditions supporting diagnosis and supplying medication as appropriate. For those who require any further training specific to the job role will provided by the Company. The post holder will work collaboratively across the service to meet the needs of patients together with colleagues internally and externally. Providing support to the GP practices and other members of the service where appropriate. The post holder will deliver care within the boundaries of their role and competence.
The successful candidates will have a good understanding of the health and social care system and whole system delivery. You will have evidence of continuing professional development.

Main duties of the job
To assess, diagnose, treat, refer or signpost patients/service users who require a home visit with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
Use advanced clinical skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.
