PCN Care Homes - Nurse Practitioner

Job summary

This is an innovative, clinically led Care Homes Service. The role is essential to support the clinical lead within the team and act as a deputy in their absence, supporting day-to-day organisational and clinical decisions.

Main duties of the job

The post holder is an experienced nurse, with a minimum of 3 years post registration training, who acts within their professional boundaries, will provide care for the patient from initial history taking, treatment and evaluation of their care. They will demonstrate safe clinical decision-making and reasoning and care for patients within care homes on behalf of the PCN practices. They will work collaboratively with the multi-disciplinary care homes team and GP team to meet the needs of patients, supporting the delivery of the Enhanced Health in Care Homes DES.

The post holder will be able to perform comprehensive assessments of older people living with illness or long-term conditions which affect their daily lives. This involves close inter-disciplinary working with nurses, therapists, pharmacists, dietitians, social workers and many other health and care professionals. In addition, working closely with GPs, Old Age Psychiatrists, hospital clinical specialists and frailty services to ensure that older people receive the highest possible levels of care. The post holder will ideally require an understanding of the comprehensive geriatric assessment toolkit as per the British Geriatric Society but this is not essential as training will be given.

We operate across the Dover and Folkestone areas and eight GP practices form our PCNs. Our PCNs are led by a Clinical Director, who is a GP, supported by a team of around 50 highly qualified health professionals.

Job responsibilities

Develop individualised treatment and management plans using evidence based practice and proactively monitor patients and review care plans.

To identify and plan preventative measures and anticipatory care needs.

To make direct referrals on to other services or professions, discuss diagnostic tests and instigate treatments and interventions as required working collaboratively with the MDT and patients GP.

To analyse signs and symptoms, laboratory tests and other measures of function, in formulating a diagnosis.

To work as an autonomous practitioner, managing and prioritising own caseload, through clinical judgement, according to local and national guidelines and identified need.

To work in a variety of settings, exercising a high level of judgement, discretion and decision making in clinical care.

To continuously review practice and participate in implementing change as appropriate and defined within the organisation policy

To understand and or have experience with advanced care planning decisions/discussions around resuscitation and escalation using DNAR, TEP and RESPECT.
