Job summary

Devon Health Inequalities Fellow - 8 posts available (Plymouth-3, Torbay-2, Devon-wide 2, Exeter-1) Exciting opportunity for 8 GPs with an interest in Health Inequalities. 8 Posts, each for 1 session per week. Fixed term contract 12 months Pay based on standard NHS WTE fellowship scale, £8500 per session with additional £1000 educational allowance.

Main duties of the job

Devon Training Hub are looking to appoint 8 GP Fellows in health inequalities to: -Upskill to meet patient needs in areas of high deprivation/serving diverse communities -Undertake project work on reducing health inequalities at a PCN level to align with ICB priorities for health inequalities, examples might include: refugee/asylum seekers, trauma-informed care, digital inequalities, obesity, substance misuse/deprescribing and Core 20 +5 -Collaborate with systems partners to deliver on One Devon priorities to reduce Health Inequalities.

Devon Training Hub is an organisation responsible for the training, education and development of staff at all levels in Primary Care in Devon. We are a small and friendly team based in Plymouth but with team members based in Devon, Cornwall & Somerset and we offer the ability to work flexibly at the role requires.

About your role

Engaging People/Key Working relationships

  • Apply systems thinking approach to identify and work with all key stakeholders in the given locality, including Primary Care Networks, ICB and voluntary sector
  • Liaise with the Clinical Leadership Team at Devon Training Hub to ensure good exchange of plans, ideas and information
  • Attend local peer learning groups and action learning sets and optional participation in the national education programme relating to health inequalities
  • Work with multi-professional members of the PCN team to support their understanding of Health Inequalities and their role in reducing them
  • Liaise with previous and existing health inequality fellows to share learning and develop existing initiatives where appropriate

Delivering Results/Functional Responsibilities

Identify issues at PCN level pertaining to Health Inequalities and develop a quality improvement project to address this

Write up and present your project in line with agreed guidelines from the Devon Training Hub

Agree an educational professional development plan with the Devon Training Hub Clinical Leadership Team, identifying your own learning needs and areas you might like to upskill

Participate in and support the delivery of Health Inequality related events in Devon

Disseminate learning about how to reduce health inequalities across the Primary care network and regionally where appropriate

Management and leadership

Use leadership skills across health care boundaries (both environmental and professional)

Working with health organisation leads and system partners to develop projects that support system needs

Liaise with Primary Care leads and other interested parties to share learning and outcomes of projects and fellowship as whole

Utilise your position and your relationship with system stakeholders/networks to raise awareness of health inequalities across Devon

Being a positive advocate for Devon Training Hub and vulnerable/diverse communities in Devon

Setting Direction and Service Improvement

Working ICB Health Inequality leads to ensure projects and activity align with system priorities in reducing health inequalities

Using QI methodology to assess service improvement delivered by project

Ensure any successes or learning about service improvement are shared with other PCNs/Stakeholder for wider impact
