PCN Clinical Pharmacist

Job summary

Are you looking for a new challenge in Primary Care? Macclesfield Primary Care Network is looking for a Clinical Pharmacist to join the team to work across all six medical practices based in Waters Green Medical Centre.

The Clinical Pharmacist will have a key role in supporting delivery of the new Network Contract DES Service specifications and will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care.

Main duties of the job

The candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews. The successful applicants will work as part of a multidisciplinary team within their Primary Care Network.

Experience in General Practice would be an advantage but is not essential if the applicant is clinically up-to-date and understands the process of a full medication review in the primary care setting.
