Job summary
Macclesfield Primary Care Network is an innovative and friendly PCN that is looking for an Advanced Nurse Practitioner(s) to enhance the team, and the quality of our patient care.
The ANP Prescriber would work across our six GP practices, all located at Waters Green Medical Centre, helping to support the acute, on-the-day need of our patients.
Working within their scope of practice, the ideal candidate will manage a daily caseload of patients and deal with a wide range of health needs, ensuring the highest standards of care for all of our patients.
You will use your advanced autonomous clinical skills and in-depth knowledge base as part of a committed and welcoming multidisciplinary team, supporting with the delivery of the NHS Network Contract Directory of Enhanced Services (DES).
You will need to be enthusiastic, forward thinking, and motivated. Excellent communication skills and a commitment to primary care, with the opportunity to increase your skills as well as apply them in a primary care setting.
Macclesfield Primary Care Network is based in Waters Green medical Centre. The medical centre houses all 6 local GP practices and serves a population of 62,000. The Macclesfield PCN is forward thinking, patient driven and dedicated to improving services for patients in the Macclesfield area.
Main duties of the job
- Act as an autonomous practitioner, working both indepently and in conjunction with other health care professionals, to provide holistic care for the presenting patient from history taking, clinical assessment, diagnosis, treatment and evaluation of care.
- Assess, diagnose and treat patients attending primary care services with a range of acute, non-acute, and chronic medical conditions.
- Be a key member of the Acute Multidisciplinary Team and support the team and GPs in the delivery of primary care as part of the acute service, in operation Monday Friday during working hours.
- Assess, diagnose and treat patients in our Urgent Care Hub who require acute medical attention, and refer appropriately in line with local and national guidance.
- Demonstrate safe, clinical decision-making and expert care, providing professional advice to patients whilst always maintaining clinical excellence.
- Work collaboratively with the PCN multidisciplinary team and support the development of new and innovative models of care to meet the need of the PCN patient population.
- Prescribe independently in line with the formulary.
- Use advanced clinical and communication skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.
- Care occasionally may need to be provided in other settings as required.
