Local Care Direct is a non-profit Social Enterprise operating across 24 hours a day, 7 days a week, 365 days a year, to provide various primary and urgent health care services across West Yorkshire.
We are looking for an Advanced Clinical Practitioner - Telephone Triage.
Salary: £29.02 per hour - £37.73 out of hours*
*(In Hours = Monday to Friday 08.00 to 18.00 &Out of Hours = Monday to Friday 18.00 to 08.00 and all day Saturday Sunday)
Location: Local Care Direct, Unit 2, Longbow Close, Pennine Business Park, Huddersfield, HD2 1GQ
Shifts Available: Shifts Available working across a 4 week rolling rota:

Main duties of the job
- The
advanced clinical practitioner will support the Urgent Community
Response (UCR) services
mission to deliver interventions within a 2-hour timeframe, either through
face-to-face visits by partner clinicians or remote management.
- The advanced clinical practitioner will be an
integral member of the clinical team to deliver high-quality patient care, that
involves utilising technology to conduct remote consultations for patients
referred into the service.
- The advance clinician practitioner will have a
strong interest in complex triage and is skilled in remote consultations and
confident in managing acutely unwell patients. The advanced clinician
practitioner will strive to make a real difference by helping patients stay
independent and avoid unnecessary hospital admissions.

- Evidence of experience of triage, by telephone or face-to-face
