Job summary
This
isnt your average healthcare assistant role, as a band 4, your skills would be
used to clinically and socially review patients and offer support and advice to
patients and their loved ones.
You
would become an integral part of our exceptional community team, working to
promote and maintain patients wellbeing, all under the guidance of the Hospice
Specialist Nurses and multidisciplinary teams. The successful candidate will be
given the support and training required to meet the varied elements of the
role.
The role requires you to carry out your
duties according to the Hospice philosophy and values, complying at all times with policies around the safeguarding of
vulnerable adults and children, whilst maintaining confidentiality of patients
and loved ones.
Main duties of the job
The successful candidate will work
closely under the guidance of the nursing team, monitoring patients physical,
psychological and spiritual needs. Promoting patient wellbeing and
independence. Whilst assessing, signposting and supporting their loved ones
under the guidance of the family support team.
You will work as an integral part of The
Hospices Integrated Community Team, making a lasting difference to the lives
of people through care that will have a positive impact on the patient, their
loved ones and the community for many years to come.
Represent the organisation and uphold the
organisations reptation and values, supporting income generation activity.
Fundamentals
of the role:
Routine
home visits and phone calls to patients and carers to review symptoms and
wellbeing, using a validated assessment tool.
Referring
to appropriate external clinical and social services.
Working
with the Hospice multidisciplinary team to ensure patients are
able to
remain in their preferred place of care to end of life.
On
occasion working with The Hospices Rapid Personalised Care Team delivering
personal care to those at end of life.
Working
under the support of the carers team to assess and support informal carers,
offering emotional and practical support.
Recording
all activities on System 1, our electronic database in accordance with Hospice
policy.
Lone
working, following Hospice policy and guidelines.

Demonstrate
and maintain consistently high standards of individualised palliative nursing
care and support under the guidance of the Community Nursing Specialist.
Work
alongside the patients family and / or professional carer, maintaining an
approach that incorporates physical, psychological, social and spiritual
aspects of care, in accordance with the Hospice of St Francis philosophy.
Observe
and report changes in the patients symptoms, condition and circumstances to
the Hospice Community Nursing Team, notifying other healthcare professionals
as necessary.
Assess
patients physical and emotional needs, utilising assessment tools such as
IPOS and under the guidance of the Community Nursing Specialists offer basic
symptom control advice and support and review outcomes.
Independently
manage, with remote support from community nursing team, patient and carer
emergencies within the community setting. Maintaining a calm and professional manor whilst acting
in a timely and effective way.
To
take responsibility for the ongoing review of a small group of patients,
reporting to community nurse specialist when appropriate for advice and
guidance, escalating concerns appropriately within the Hospice and wider
community MDT.
Maintain
concise accurate and legible records of patient/family care in all health
care settings, using electronic patient data base, meeting legislative
requirements within The Hospice of ST Franciss practices and procedures.
Responsible
for identifying and acting on any risk of safeguarding to patients and their
families in accordance with the Hospice of St Francis Safeguarding of Vulnerable Adults and the
Safeguarding of Children Policies and Procedures.
Offer
bereavement support, either face to face or over the phone, referring as
appropriate to the bereavement team and highlighting any bereavement concerns
to The Community Nursing Specialist
Assist
with the patients nutritional requirements including the preparation of
light meals and drinks according to dietary requirements and choice.
When
required assist with patients personal hygiene and toileting, which may
include the use of bedpans, commode, lavatory, catheter or conveen.
Administer
medication in accordance with the Hospice of St Francis policy and procedure
where relevant training has been undertaken.
Undertake
simple dressings as prescribed in the patients care plan whilst maintaining
best practice in management and control of infection.
To
support patients with rehabilitation in their home setting, working on goal
setting and motivation.
Ensure
a safe environment to work within, and report in a timely manner to the
Community Nursing Services lead if concerned.
