Job summary
This role involves coaching and motivating patients in managing their own health and wellbeing to live independently and improve their health outcomes. The post holder will work with individual and/or groups of patients to deliver sessions on healthy eating, physical activity and health and well-being to meet personalised care plans.
The role works within their practices Multi Disciplinary Team; induction and training is given to help candidates integrate into Primary Care.
This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within primary care.
This is a full-time role covering 4 day at the Huddersfield Road Surgery and 1 day at Darton Health Centre.

Main duties of the job
Health coaching uses coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
You will work in partnership with your clinical and non-clinical colleagues across the PCN to ensure the role delivers the best possible outcomes for our patients.
The role is varied, and may include supporting self-management education, peer support, case management and facilitating group consultations.
You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions. Your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions.
