Job summary

We are looking for a compassionate, motivated, and skilled Assistant Practitioner (band 4) to join our Day Hospice Team, providing high-quality, person-centred care and support to patients with life-limiting illnesses.

In this rewarding role, you will play a key part in supporting patients and their families with advance care planning, ensuring their wishes and preferences are recognised and respected. You will work closely with the multidisciplinary team to deliver holistic care that promotes comfort, dignity and wellbeing.

The post holder will be part of a team committed to delivering and leading high-quality care which reflects the philosophy and objectives of St Lukes (Cheshire) Hospice. The post holder will provide support for patients/families and other members of the wider multi-disciplinary health care team involved in patient care.

To provide a professional nursing service to patients and families in the Day Hospice and across the hospice settings

Supervise and guide students, junior members of staff, unqualified carers and volunteers in the Hospice

Contribute to the ongoing development of the Hospice service in the quest to provide specialist palliative care that keeps the patient and family as the central focus

Key Tasks and Responsibilities

To work within the policies and procedures of the Hospice to maintain a safe working

environment

To assess, plan, implement and evaluate patient care under the leadership of a registered

nurse.

To practise research-based techniques in care delivery.

To participate in the introduction and evaluation of new procedures for patient care.

To always treat patients safely, with dignity, holistically and respect confidentiality.

To participate in multi-disciplinary and clinical meetings

Liaison with other professional groups involved in patient care

Contribute towards prevention and management of infections

The post holder will be responsible for the Health and Safety of patients, staff and visitors

whilst on duty

Working without direct supervision, delivering care independently, within the parameters

of practice of the band 4 role, accessing clinical advice as and when needed.

Maintenance of accurate patient care records

Report deficiencies or dysfunction of equipment

Participate in audit activity and review of standards within the Hospice

To refer to internal and external agencies.

To participate in end of life planning talks with patients and their families and advance care planning.

Education / Personal Development

Actively participate in clinical supervision

Demonstrate commitment to ongoing personal development through study, official

courses and participation in in-house meetings and courses

Acquire skills, under the Assistant Practitioner remit which will enhance patient care

Act as a role model to junior staff / visiting students and share knowledge and skills

relevant to palliative care
