Job summary

We have a number of primary care roles available, clinical and non-clinical, for those with the drive and passion to help us shape and deliver this vital service. Together with our partner providers at an accommodation centre North of Lincoln, this integrated and collaborative approach will not only deliver tangible care and support to this cohort but also ensure that our established Lincolnshire healthcare services continue to be available as normal for the wider population.

Main duties of the job

To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well person clinics, as well as actively supporting the service management team in the reviewing and delivery of clinical policy and procedure.

K2 Healthcare Ltd will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

This post is Self-employed with the fee including all oncosts related to the position.

About us

K2 Healthcare are a GP Federation initially covering 16 practices. 10 in Grantham and surrounding villages and 6 in Sleaford covering 130,000+ patients.

We are developing a diabetes pathway to improve services and access for all our diabetic patients and developing a specialist asthma pathway to improve asthma management.

We are deploying paramedics and developing paramedic roles in our primary care setting.

We are continuing to support our practices resilience through the Additional Roles Reimbursement Scheme (ARRS); employing clinical pharmacists, musculoskeletal (MSK) practitioners, occupational therapists and care coordinators, and supporting and developing neighbourhood working across our population.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  1. To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the service manage
  2. To produce the IPC annual statement in conjunction with the service manager
  3. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the service
  4. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
  5. To develop, implement and embed health promotion and wellbeing programmes
  6. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
  7. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
  8. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
  9. As required, to diagnose and manage acute and chronic conditions
  10. To prescribe medication for patients in accordance with PGDs and own scope of practice
