Job summary
The Project Manager will lead the design, development and
delivery of new and existing services across the Lincolnshire PCN Alliance,
supporting system-wide priorities and the ongoing transformation of primary
care.
Working collaboratively with clinical, operational and
system partners, the role will translate strategic priorities into practical,
deliverable service models, ensuring alignment with commissioning requirements
and the PCNA business plan.
The initial focus of the role will be to lead the
development and implementation of a new, county-wide weight management service,
designed to support patients through accessible, community-based interventions
and multidisciplinary care. This will involve bringing together partners across
primary care, community services and wider system organisations to deliver a
coordinated, outcomes-focused model that supports long-term behaviour change
and improved population health.
This is a fixed-term, full-time contract for 12 months with an anticipated start date as soon as possible. Due to service need, we are prioritising candidates who are able to start promptly, and this role may not be suitable for individuals with long notice periods.
Main duties of the job
The role will support the development and
implementation of new models of care, coordinating activity across multiple
stakeholders to ensure effective planning, delivery and embedding of change.
This includes working across organisational boundaries to enable collaboration,
align priorities and support the development of the Alliance and its member
PCNs.
The postholder will provide project
leadership, coordination and oversight, ensuring that programmes are delivered
to a high standard, within agreed timescales, and that outcomes are achieved
and sustained.

Care Networks to work collectively at scale within the wider health and care
system. As PCNs continue to mature, the focus looks towards delivering tangible
improvements through population health management, service transformation, and
stronger partnership working.
This includes supporting the development of
neighbourhood-based models of care, bringing teams together across
organisational boundaries to provide more joined-up, accessible services.
The PCN Alliance is expanding its delivery capability and is
seeking a Project Manager to lead the development and implementation of these
services, alongside supporting wider transformation programmes across
Lincolnshire.
