Job summary

As a Community Link Workers, you can be at the forefront of helping people in Glasgow improve their health and wellbeing. It is all about getting the right support to people at the right time. This role is for you if you really care; if you are very proactive; if you are great at making community connections and love working with people to find solutions.

Main duties of the job

Community Link Workers work from within GP Practises. Through 1:1 support appointments and small group work we help people change their lives for the better. We are recruiting for 2 full time (2 x 37.5 hours) Community Link Workers to work in Glasgow, or the equivalent in part time posts. Please specify your preference for full or part time in your covering letter. The service is provided Monday - Friday within GP Practice hours. Please be advised the roles are initially offered on a fixed term contract to 31st March 2024; with the potential for a permanent position dependent on contract renewal. Community Link Workers are uniquely placed third sector workers operating within GP Practises. We work with the entire patient population aged 16 and above. Whilst we are not medical staff, we bring a unique skill set and a third sector perspective that complements the work of the multidisciplinary team in the GP Practice. We support people to tackle their health and wellbeing problems and connect with community support services. Common reasons for referral to our service include low level mental health difficulties, problems with housing, poverty, relationships and bereavement. Our Community Link Workers are based in GP Practises in areas of high deprivation, where people are experiencing health inequalities. We may close this role early if a sufficient number of application are received.

  • Knowledge and experience of working with people who are experiencing complex social and emotional circumstances including mental health difficulties
  • Ability to work in a structured service, to follow a service model and collect information about the impact of the service
  • Extensive effective interpersonal skills in working with people on a 1:1 basis, including the ability to learn and use recognised therapeutic techniques Knowledge about the impact of poverty and health inequalities on health and wellbeing
  • Ability to establish professional relationships with multiple stakeholders; i.e. GP Practice teams, Social Work, Mental Health Services and Money Advice Services
