Job summary

Are you a Practice Nurse interested in working in a long established, forward thinking and dynamic Medical Practice?

You will be required to work 37.5 hours per week, Monday to Friday; between the hours of 08:00 and 18:30.

Main duties of the job

The post holder is responsible for the delivery of day-to-day Practice nursing services to the Practice population.

Supported by the clinical team within the Practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities both in Practice and in the community. They will work collaboratively with the general Practice team to meet the needs of the patients and to refer into complementary services as required; supporting the delivery of policy and procedures and providing nurse leadership as required.

Job description

Job responsibilities

The post holder will be responsible for the delivery of General Practice Nursing to the practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care. As an autonomous Practitioner, the Practice Nurse is responsible for the care delivered to patients, demonstrating critical thinking and skills in clinical decision-making in the management of Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery of a healthier Whitworth through listening and responding to Patients needs.

The post holder may, in the future, be required to attend local Network meetings and support other developing activities in the area.

Main Duties

  • Hold reviews for long term conditions i.e. COPD, Asthma, Hypertension, CHD and Diabetes;
  • Complete cytology screening;
  • Monitor QOF;
  • Delivery of vaccinations for both Adult and Children including Flu, Covid, Travel and B12;
  • Attend MTD clinical meetings, LTI and monthly Practice meetings;
  • Infection Control Lead for the Practice;
  • Complete audits;
  • Update Practice policies;
  • Order stock i.e. vaccines, clinical consumables;
  • Monitor emergency drugs and crash trollies.

The post holder must be able to carry out all of the above and have up to date training for Cytology and Immunisations for both Adult and Children.

Qualifications

Essential

  • Registered general Nurse.
  • Current NMC Registration
  • GCSE Grade A to C in English and Maths

Desirable

  • Community Nursing Specialist Qualification or Equivalent.
  • Relevant Nursing Degree.

Experience

Essential

  • At least 1 years experience of working in a similar role within a Primary Care setting; Experience of quality initiatives such as clinical benchmarking; Experience of managing patients with long term conditions; Requesting pathology tests and processing the results; Immunisations i.e. routine, childhood and travel; Understand importance of evidence based practice; Broad knowledge of clinical governance; Ability to record accurate clinical notes Knowledge of health promotion strategies.

Desirable

  • Wound Care / Removal of sutures & staples; Insight into public health issues in the local area; Awareness of issues within the wider health arena.
