Job summary

Beacon PCN wish to recruit a Full time, experienced PCN Pharmacist to join their team. Working within 1 Practice, this role gives you the opportunity to provide PCN DES services, develop your own clinical role and work with the Practice on agreed projects / work areas. Charnwood GP Network already employs 13 PCN pharmacists who are supported by 3 Senior Pharmacists and the Lead Clinical Director. We offer access to the NHS Pension Scheme and a generous annual leave entitlement. This post is for 37.5 hours per week.

Interviews will take place week commencing 24th February.

Main duties of the job

To be responsible for implementing an effective medicine management system within the practice. You will be expected to work to achieve cost-effective prescribing, work as an independent prescriber, provide advice and support for both patients and colleagues, undertake all duties expected within the changing role over time. Lead and take part in PCN audits, Federation audits, and quality prescribing initiatives. Take part in training of others when required. Maintain own professional accreditation requirements.

About us

Charnwood GP Network is comprised of 4 Primary Care Networks in the geographical area of Charnwood in West Leicestershire. In total across the 4 PCNs we have 21 General Practices covering the populations of Thurmaston and Birstall in South Charnwood through to Loughborough and Shepshed in North Charnwood.

This role specifically will be to work as part of Beacon PCN.

Job responsibilities

The following are the core responsibilities of the Clinical Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Practice as an independent prescriber working within scope of practice (when qualified) To provide medication review services with patients in each PCN by telephone calls, clinical records reviews, face to face reviews or during domiciliary visits. To reconcile medicines following patients discharge from hospital or clinic appointments. Review all safety alerts ensuring the required actions are taken. Carry out targeted medication reviews for high-risk patients (eg by using eclipse). Review and implement safe prescribing systems for vulnerable patients, improving compliance. To provide subject matter expertise on medication monitoring, implementing and embedding a system. Undertake practice/ PCN and Federation level audits.

Essential

  • GPhC Registered Pharmacist

Desirable

  • GPhC Independent Prescriber Qualification
  • Minor Ailments certification
  • Medicines Management Qualification
  • MUR and repeat dispensing certification

Experience

  • Minimum of two years working as a pharmacist
  • Experience in managing pharmacy services in primary care
  • Broad knowledge of General Practice
