Job summary
The Practice Nurse is responsible for the
delivery of nursing services to the practice population. Supported by Senior
Nurses the post holder will deliver high quality nursing care within their
scope of competence, focusing on supporting patients to manage their long term
conditions, address health issues and promote health by delivering education
and screening activities.
Main duties of the job
The post holder will work collaboratively with the multi-disciplinary team to meet the needs of patients and will play an integral part in delivering the key performance indicators for the organisation.

Job responsibilities
Manage Health Interventions
- Assess, plan, implement and evaluate programmes of care for patients that
promote health and wellbeing, including those with long-term conditions.
- Pro-actively implement programmes of screening for patients.
- Administer adult and child immunisations and vaccinations in accordance
with national and local programmes. Competence
in anaphylaxis, resuscitation techniques and other mandatory training as
decided by the organisation.
- Undertake various nursing procedures including venepuncture, urinalysis,
new patient health checks, annual health checks, BP checks, ear irrigation,
pregnancy testing, stitch/clip removal, and wound management.
- Provide health promotion advice and information to support positive
lifestyle changes.
- Recognise, assess and refer patients presenting with mental health needs
to an appropriate clinician and recognise the need for crisis response services
(referring where appropriate).
- Recognise and understand the importance of observing, documenting and
communicating adult and child safeguarding concerns.
- Make appropriate referrals to other members of primary, secondary and
community care teams as appropriate and within own competence.
- To
assist in the management of long term conditions, including cardiovascular
disease, diabetes, asthma and COPD management.
- Assist
in the process of diagnosis with patients suspected to have a chronic disease,
referring to other clinicians as appropriate.
- Assist
in the management of programmes of care for patients with chronic diseases by
planning, providing and evaluating care under agreed local/national guidelines,
protocols and patient group directions.

in the management of QOF & KPI reporting for the practice on an annual
basis.

to NMC, local and national policies, guidelines and codes of practice where
appropriate.
Promote Health
- Proactively
works with the community and other statutory and voluntary agencies empowering,
enabling, encouraging and supporting individuals, families and groups to
address issues which affect their health and social well-being.

Communication
- Uses
developed communication, negotiation, conflict management skills which require
empathy, reassurance and persuasive skills where there can be significant
barriers to acceptance which need to be overcome.
- Keep accurate, contemporaneous documentation and care plans in
accordance with the Nursing and Midwifery Council, local and national
guidelines.
- Develop effective communication links with other professionals and
agencies across all boundaries, at all levels of the organisation including
users, carers, social care and the voluntary sector to insure effective
delivery of care that provides the best outcome possible for service users.
