Registered General Nurse (Queenscourt Connect/Queenscourt at Home) Band 5

£27,193 - £32,992 per annum/pro rata (Dependant on Experience)

37.5 hours per week, Permanent

Location: Southport, Merseyside

We are seeking a Registered Nurse at Queenscourt Hospice to work across both Queenscourt Connect and Queenscourt at Home. Reporting to the Sister or Ward Co-ordinator/Queenscourt at Home Co-ordinator, you will be responsible for the assessment of care needs, the development, implementation, and evaluation of care, following the District Nurses care plan in the patient's home. You will communicate sensitively with patients, families, and health professionals, attend all Queenscourt MDT meetings, and participate in the delivery of Carers Programme, Breathlessness management Programme, and Bereavement support, liaising with members of the Primary Care Service, external agencies, and Queenscourt Hospice Staff. You will also supervise and coordinate the work of other staff and volunteers as needed.

Main duties of the job

To be successful in this role, you must be a Registered Nurse with community nursing experience, able to demonstrate effective communication skills. You should also have experience using patient information systems and Microsoft Office.
