Job summary
We have a vacancy for a 12 month Fixed Term Health and Rehabilitation Support Worker to work as part of the North West West Community Team, covering Lincoln/Gainsborough and surrounding areas.
Hours:
30 hours per week
Contract:
Fixed term for 12 months
Starting Salary:
£22,294, (band X1) per annum pro rata (plus weekend unsocial hours enhancement of 30%)
Salary Increment:
Band X receives an annual increment pay rise.

Main duties of the job
You will work as part of a multi-disciplinary team to provide a high standard of compassionate, patient-centred palliative care. This includes practical, rehabilitative, and emotional support which will be delivered under the direct or indirect supervision of a Registered Nurse or Allied Health Professional.
Qualifications and Experience:
Applications are welcome from candidates with previous or current experience in delivering person-centered holistic care in a variety of settings with an interest in palliative and end of life care. Experience in the promotion of patient Wellbeing and supporting people in achieving their goals would be desirable.
