Main duties of the job

As an ACP, you will provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health. You will diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills, and prescribe and manage medications as an independent prescriber. You will also coordinate care across services, act as a referral agent and care coordinator to ensure seamless patient journeys, lead on clinical quality and safety, mentor and support colleagues, and safeguard vulnerable individuals.

About the Role

As an ACP, you will:

  • Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.

  • Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
  • Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
  • Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
  • Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
  • Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.

What We Expect

  • Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
  • Advanced assessment and minor illness qualifications (Level 6 or 7).
  • Independent prescribing qualification (Essential).
  • Proven experience in autonomous practice and clinical leadership.
