IC24 is seeking an experienced Advanced Clinical Practitioner to join our Primary Care team and make a real impact in the heart of the community.
This is a senior clinical role where youll work autonomously, using advanceddecision-making skills to deliver safe, effective, and patient-centred care across urgent and primary care settings.

About the Role
As an ACP, you will:
- Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.
- Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.
- Prescribe and manage medications as anindependentprescriber, ensuring safe and effective treatment plans.
- Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
- Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
- Lead on clinical quality and safety, promoting best practice,supportingaudits, and contributing to service improvement initiatives.
- Mentor andsupport colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
- Safeguard vulnerable individuals, applying policies and legislation inchild protection, mental health, and safeguarding adults.
- Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.

- Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
- Advanced assessment and minor illness qualifications (Level 6 or 7).
- Independent prescribing qualification (Essential).
