Were offering an exciting opportunity for a Mental Health Practitioner to join our forward-thinking and compassionate Primary Care Network (PCN). Youll be supporting five GP practices across Deans and Central Brighton, delivering high-quality, person-centred care right where its most accessible in general practice.
Main duties of the job
What Youll Be Doing: Hold a personal caseload of patients with mild to moderate mental health needs Offer directly bookable appointments and provide brief evidence-based interventions Deliver continuity of care through follow-up and longer-term support within GP practices Provide early identification, support, and management of psychological distress and mental illness Work as a bridge between primary and secondary care, referring only when clinically appropriate Play a crucial role in reducing pressure on GPs by offering dedicated mental health support Participate in clinical supervision, safeguarding procedures, and continuous professional development

What we are looking for: A qualified mental health professional (e.g., RMN, social worker, psychological therapist) Skilled in providing short-term and ongoing mental health support in a community setting Comfortable working autonomously and as part of a multi-disciplinary team Committed to inclusive, compassionate, and non-judgmental care Confident with digital record-keeping, safeguarding protocols, and managing risk appropriately
