As an Urgent Care Practitioner, youll be expected to assess all patients of all ages and provide the right management or referral. Youll have to be reliable and dependable, passionate and committed to delivering high quality health care in an out-of-hours (OOH) environment.

If you have advanced Physical assessment (level 6 or above) minor illness course (level 6 or above), palliative care/ end of life care experience and would like to join our professional team as an Urgent Care Practitioner (UCP), then we would love to hear from you. As we provide an OOH service, you must be willing to work evenings and weekends.

Main duties of the job

You will be responsible for diagnosing and treating patients with minor illness and other urgent presentations face-to-face within our out of hours services. You don't need to be a prescriber as you'll be able to work from a range of agreed PGDs. You will usually be based at one of our out of hours locations within Norfolk and Waveney in order to make home visits to patients in an effective timely manner.

This role is particularly suited to an experienced autonomous Clinician that has undertaken core modules of advanced practice within the Specialist Paramedic, Nurse/Paramedic Practitioner program or an experienced Nurse/Paramedic who has additional training in assessing minor illness and physical assessment. This is an opportunity like no other one that makes a difference to patients that require urgent care or deteriorate during the OOH period.

Locations Across Norfolk & Waveney: we have face-to-face base locations in Norwich, North Walsham, Dereham, Thetford, Kings Lynn, Fakenham, Great Yarmouth, Beccles, & Lowestoft

Hours AvailableFlexible shifts, including evenings, overnights and weekends, minimum 12 hrs per week.

Shifts AvailableTues to Fri 18:30-06:30 and 08:00-22:00 on weekends

Whats in it for you?

You will be responsible for the assessment of health and clinical needs, utilising professional clinical judgement. This requires generalist and specialist skills to assess and deliver care to patients across all age groups out of hours. 

You will provide assessment, diagnosis, treatment and discharge of a range or urgent medical concerns and plan and deliver the most appropriate care for them. 

You will be responsible for providing face to face and occasional telephone consultations for patients presenting with unscheduled primary care conditions and minor illness needs. 

Other issues may include palliative and end of life care, medication advice and health information supporting individuals to access the appropriate level of care including advice to manage their symptoms at home. 

You will be responsible for the supervision, teaching, precepting and mentoring of other colleagues who may include students and other clinicians. 

You will be expected to support delivery of the national healthcare agenda and represent the organisation in a timely and professional manner as and when required.
