Do

passion for innovation in healthcare? South Hardwick PCN is seeking an
experienced and visionary Digital Transformation Lead to spearhead the
development and implementation of a digital strategy that meets the 2025/26 PCN
DES requirements and aligns with the NHS 10-Year Plan for digital
transformation in primary care.
This is a pivotal role for a dynamic leader who can drive digital change across
our network of practices, improve patient outcomes, and enhance operational
efficiency. You will work closely with the PCN General Manager, Practice
Managers, and wider multidisciplinary teams to embed digital solutions that
support sustainable, high-quality care.
Main duties of the job
The
Digital Transformation Lead will be responsible for designing and delivering a
PCN-wide digital strategy that supports clinical priorities, enhances patient
access, and ensures compliance with national digital health mandates. The role
will involve leading digital innovation projects, supporting digital literacy
across teams, and ensuring the effective use of digital tools and data to
improve care delivery.

- Develop
and implement a PCN-wide digital transformation strategy aligned with the
2025/26 DES and NHS 10-Year Plan.
- Oversee
the implementation of digital tools, including digital triage systems,
population health management solutions, and business intelligence platforms.
- Act as the digital lead for the PCN, representing the network at both internal
and external stakeholder meetings.
- Collaborate with clinical and non-clinical leads to embed digital solutions
into care pathways and operational workflows.
- Identify and secure funding opportunities for digital innovation.

Project Management
& Implementation
- Oversee the delivery of digital projects from concept to implementation,
ensuring timely and effective execution.
- Coordinate with ICB, neighbourhood and system partners, to ensure
interoperability and integration.
- Monitor project outcomes and report progress to the PCN Board and
stakeholders.
Workforce
Development & Digital Literacy
- Support staff across the PCN to develop digital competencies and confidence in
using new technologies such as the NHS app.
- Deliver training and resources to promote digital inclusion for both staff
and patients.
