Job summary

South Hardwick PCN is seeking a passionate and motivated Pharmacy Technician to join an exciting and forward-thinking primary care network with ambitions to truly transform patient care.

Pharmacy technicians play an important role for the practices of South Hardwick PCN by complementing Clinical Pharmacists, Practice nurses , GPs, administrative practice teams , and other members of the PCN multi-disciplinary team.

With the flexibility to work full or part-time, this is an exciting opportunity for eligible candidates looking to expand their horizons and embark on a new challenge in an innovative PCN

The post holder will develop and support effective medicines optimisation & reconciliation processes; contribute to personalised and anticipatory care; help implement effective prescribing policy & practice across the PCN and wider system.

It is anticipated that the successful candidate will work across 1-2 practices within the PCN.

Main duties of the job

Job Purpose

The purpose of the role is to participate in the implementation of strategies to maximise achievement and monitoring of Investment and impact fund, PCN national requirements, local guidelines, and national guidelines (NICE) implementation and utilisation of medicines optimisation initiatives.

The post holder will help patients to get the best from their medicines by implementing agreed and approved protocols, around medications, including anticoagulation, cardiovascular medicines and inhalers.

In addition, the post holder must be willing to enrol and complete any nationally mandated training for PCN pharmacy technicians if you have not already done so.

Clinical

Ø Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

Ø Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation.

Ø Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

Ø As determined by the PCN, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies

Ø Support the Clinical Pharmacist in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR

Ø Provide expertise to address both, the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

Ø Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

Ø Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Ø Lead on clinical searches and monitoring of data

Ø Lead the PCN Pharmacy team in respect to developing policies and procedures

Ø Contribute to the design and delivery of patient questionnaires or pre-review resources to refine the system for medication reviews

Ø Collate and filter information from the patient to be documented on the clinical system or escalated to a Clinical pharmacist or other relevant clinician in the practice, if appropriate.

Ø Support practice Prescription's teams in streaming general prescription requests, to allow GPs and clinical pharmacists to review the more clinically complex requests.

Ø Support the implementation of local and national prescribing policies and guidance within GP practices, care homes and other primary care settings.

Ø Support the Primary Care Network to deliver on contractual agenda, support Practices with QOF and locally commissioned enhanced services.
